Demographic Confirmation Forms
Demographic Confirmation Form 2021/2022
As the school year comes to an end, we would like to inform you that it’s time to update your child’s A.9 2021-2022 Demographic Confirmation Form for the year 2021-2022. Having the most up-to-date information about our students ensures that we receive all available funding to support your child's learning as well as current information to reach you during the day if necessary. The annual consent forms also allow your child's full participation in all activities, field trips and events for optimal learning at Our Lady of the Angels Catholic School.
We ask that you please complete this form in Powerschool
You can follow these steps to access, review and submit the form:
1. Log into Powerschool at https://powerschool.eics.ab.ca/public/
2. Under the “Forms & Fees” menu click “School Engage Student Forms
3. Students - Select your child’s name (please complete one form for each child)
4. Click on the word “New” beside the form name (A.9 2021-2022 Demographic Confirmation Form.

Vegetable and Pollinator Gardens
Thank you Canadian Wildlife Federation, The Fort Saskatchewan Multicultural Association, and the Multicultural Youth Group Environmental Project for your partnership and for planting our Pollinator Garden! Thank you to OLA's village who donated and picked up soil, built the garden boxes and filled them with dirt and planted the vegetables...so grateful.
A special thank you to Donamarie Lukie for her leadership and coordination to make it all happen!

Daily Reminders
- taking the time to complete the Alberta Health Service Daily Checklist
- checking your child's backpack for a water bottle. There are refill stations at the school. The water fountains remain off to follow COVID Protocols.
- placing extra masks in your child's backpack in case they forget one, it falls on the ground or it breaks. Masks are also needed in music class.

1MW - Power Options - Making Bird Nests with Mrs. Lukie
Games Day Volunteers Needed
Welcome back parents! We are looking for a few volunteers to come to the school and help us with our Games Day on Monday, June 21st!
Between 9am and 10AM we ask for help setting up the games (all the equipment and instructions will be ready for you), then you are invited to watch and interact with the staff and children (socially distanced of course :-) while they participate in Traditional Indigenous Games as well as Modern Day Track and Field Games. After the games, thank you for helping to clean up the equipment around 1:30pm.
This would truly lighten the load for staff and make it fun for children to have you there!
We are so appreciative in you are able to help!
