Nurse's Message
Please be reminded that for entrance on the first day of 7th grade, your child must have evidence of having received 1 dose of Tdap (tetanus, diphtheria, acellular pertussis) and 1 dose of MCV (meningococcal conjugate vaccine). If documentation is not received by the first day of 7th grade, students will risk exclusion from school until it is received. If you haven't already, please call your doctor as soon as possible and schedule an appointment to have these immunizations administered before September 8, 2021. Typically immunizations do not require a full appointment slot, and you can request a "nurse only" or "vaccine only" appointment. Discussing this option may help in scheduling an appointment sooner.
If you do not have insurance, your insurance does not cover these immunizations, or your child is receiving medical assistance, your child may be eligible to receive these vaccinations through the Montgomery County Office of Public Health. Please visit their website for further information - appointment dates and times are limited!
There are the following exemptions to these requirements: medical reasons, religious beliefs, or philosophical/strong moral or ethical conviction. If you would like to claim an exemption on religious, philosophical or moral grounds, please complete this form and return to school, attn. school nurse. Even if your child is exempt from immunizations, he or she may be excluded from school during an outbreak of vaccine preventable disease.
