Here is some more information on SB 1153. Please let me know if you have any other questions after reading the materials.
"The commissioner by rule shall require each school district and open-enrollment charter school to annually report through the Public Education Information Management System information regarding the total number of students enrolled in the district or school to whom the district or school provided aids, accommodations, or services under Section 504, Rehabilitation Act of 1973 (29 U.S.C. Section 794), at any time during the year for which the report is made."

Region 10 receives the CESD Sample Forms a couple of weeks after the November Conference which was this past week. I will send them out to all of the District/Charter School Section 504 Coordinators that I have listed for 2017-18 school year. I also send the guideline for usage whenever I send the email out which I am required to do.
Important Note: At the conference this year, Dave Richards announced the paper forms will only be available up to the 2019 school year. After 2019, the forms will only be available electronically through Success Ed. The reason for this change is because people are not using the forms according to the guidelines and they would like for the forms to be part of the school's system to review and adopt and not be static. Districts and LEAs will have two years to prepare for this change.

2018 LRP's National Institute on Legal Issues of Educating Students with Disabilities
The 2018 LRP National Institute will be in Dallas next year. It is expensive, but one of the best conferences that I have attended that addresses many areas of special education including Section 504.
