COVID-19 Reporting Form
As we move into the second half of the school year, we wanted to share another reminder along with a reporting form related to the COVID-19 procedures and information:
As shared within the updated isolation and quarantine guidance from the health department, a reminder that the school and health department recommend that an individual who tests positive for COVID-19 wear a mask on days 6-10, if they are no longer isolating.
We are utilizing a NOSD COVID-19 Reporting Form (see link below) to simplify and streamline reporting. If your child or a household family member has symptoms of COVID-19 and/or tested positive for COVID-19, please complete this form as your first step. Also remember to call your school secretary to report any absence. This simple form will allow a member of our team to reach out to you with your child’s specific return dates.
(sent in an email 1/18/22 to families)
'Tis the season for colds, flu, & sore throats...
Please monitor your child daily for illness. We have many students sick with colds, flu, sore throats, strep, stomach flu, etc. Reminder per the handbook: if they have a fever or stomach issues, please keep them home. Wisconsin Department of Health Services:
DHS Recommends COVID-19 Vaccine Booster Dose for Anyone 18 and Older
On November 19, the Wisconsin Department of Health Services (DHS) announced our recommendation that everyone ages 18 and older receive a booster dose of COVID-19 vaccine following the completion of their primary series. People 18 and older who have received Pfizer or Moderna may get their booster six months after their second dose, and people who have received Johnson & Johnson may get their booster two months after their single dose.
Boosters doses are an important tool in making sure protection against COVID-19 remains high. And, getting a booster dose is easy! You can get it where you received your primary series, or you can go to any other convenient location. Also, booster dose “mixing and matching” is allowed, so you do not need to get the same vaccine type for your booster dose. To find a location near you, visit Vaccines.gov or call 211.
For additional information about booster doses, additional doses, and help accessing your COVID-19 vaccine record to determine when you may be recommended for a booster, visit the DHS Additional Doses and Booster Doses webpage.
Wisconsin Department of Health Services:
This information is directly from the Wisconsin Department of Health Services:
Wisconsin children ages 5-11 years old are now eligible to receive the Pfizer COVID-19 vaccine. The Pfizer COVID-19 vaccine requires two doses for full protection and is the first and only vaccine currently available for this age group.
After successful clinical trials and a rigorous review of all available data, the U.S. Food and Drug Administration (FDA) authorized Pfizer’s request to expand the emergency use authorization (EUA) to include children ages 5-11 on October 29. On November 2, the CDC Director officially confirmed the vaccine is safe and recommended the vaccine to this age group.
To find vaccination locations currently offering the Pfizer pediatric vaccine, go to Vaccines.gov. You can also call 211 for help finding a healthcare provider or a vaccination location.
Parents and guardians can find information they should know about COVID-19 and vaccines for children on the COVID-19: Resources for Parents and Guardians page.
