5Essentials Survey Information
Parent Survey
Fremd High School is reaching out to our families and asking you to participate in the annual Illinois 5Essentials Survey. Your perspectives can help guide our school’s improvement efforts. The survey assesses perceptions of five components of school success:
- Effective leaders
- Collaborative teachers
- Involved families
- Supportive environment
- Ambitious instruction
The parent survey will be available online in English and Spanish from January 24, 2023 through March 31, 2023, at survey.5-essentials.org/Illinois. It takes about 10 minutes to complete. Note that the survey will time out and you will need to start over if you don’t finish in one sitting.
The survey is administered by UChicago Impact at the University of Chicago on behalf of the Illinois State Board of Education. If at least 20 percent of parents at our school complete the survey, a supplemental parent report will be generated and shared with us.
We are hoping to meet the target this year.

Black History Month Begins This Week!
The first day of each week during Black History Month (BHM): Show up and Show out! Wear Black History Month gear or clothing that represents Black American Culture! Watch for activities throughout the month that will celebrate Black History in a variety of ways.
Activity: Wednesday, February 1st: “Black Stars”
There are Black American Stars everywhere and today we will recognize them for always shining bright! Come ready to share the name of an influential Black American. You will write it on a star and then it will be displayed on the wall outside of the cafeteria.
All of the stars will be organized in a way to look like the Big Dipper. Do you know why? If you do not, stop by the event to learn more about Black American culture.

Young Hearts for Life Information
Young Hearts for Life - Sign up to be screened today!

So far, 221 students are registered -there is still time to sign up!!
YH4L is a FREE heart screening called an electrocardiogram (ECG) to identify high school students at risk for Sudden Cardiac Death (SCD). Detailed information on the program is found on this Parent Packet link
More than 60 young adults die each week in the United States from SCD. Since 2006, YH4L has screened over 280,000 students and has identified more than 3,000 with previously undiagnosed heart conditions. Please be aware that repeat ECG testing is recommended every two (2) years.
A simple, painless, and non-invasive ECG can detect approximately 70% of the heart conditions that are associated with SCD, while a pre-participation physical can only detect 4% of these conditions. The false positive rate for the YH4L program is less than 2%. Additional evaluation and testing by your physician may be recommended based on the results of the screening.
All students will hear information in their Wellness classes on January 20 about our upcoming Young Hearts for Life screening event to be held on February 14. We encourage every family to register their child for this life-saving screening.
Young Hearts for Life - We need family and community volunteers!
We are looking for many volunteers to assist with our YH4L screening event on February 14. Volunteers must be at least 18 years of age (current high school students are not eligible to be a volunteer). Shifts are available in the morning, afternoon, or full day. Continental breakfast items and lunch is provided to volunteers.
Volunteers assist with checking in students for their appointment, helping get them to each of the stations, assisting with the ECG process, etc.
So far, we have about 87 volunteers signed up, but we are looking to have nearly 200! Please consider donating some of your time to this important health and wellness event!
