Steps to turning in this Background Check Form:
- Print & Fill out the following PDFs (make sure to fill out both forms!)
- Turn in the PDFs into the Tate CLS Desk (Let the desk attendant know that you are turning in the forms for FIMRC HealthyKidz Program)
**If you have any trouble with printing out the forms, we will have some printed copies at the CLS desk just in case!**
