Examinations must be dated within the previous twelve months to be considered current and must include a certificate of proof of immunization. Per state requirements, proof of the following immunizations must be on file for your child (prior to entry in the 7th grade): 3 doses of Hepatitis B, 4 doses of Polio, 2 doses of MMR, 2 doses of varicella and 1 dose of Tdap (105 CMR 220 000).
If you are unsure if you have submitted a copy of a recent physical exam with proof of the required vaccinations, please contact our nurse, Michelle Riley RN at 978-649-3115 option 4.
