NEW!!! COVID 19 SECOND DOSE Vaccination
Family Communication: COVID 19 SECOND DOSE Vaccination
Hello CMS families,
On Wednesday, May 19, 2021 and Wednesday, May 26, 2021 the Centennial School District provided transportation for students and families to the Oregon Convention Center (OCC) to obtain the Pfizer vaccine. SECOND DOSE transportation will be provided this upcoming week on Wednesday, June 9, 2021 and then again on Wednesday, June 16, 2021. The times and locations are as follows:
If you received your first vaccine dose on May 19, 20201 your second dose is scheduled for Wednesday June 9, 2021.
If you received your first vaccine dose on May 26, 20201 your second dose is scheduled for Wednesday June 16, 2021.

Regarding District provided transportation, please note the following:
You will need to have a parent or guardian sign a permission form, unless they will be accompanying you on the bus.
Due to the special nature of this opportunity, students with medical needs that require specific adult assistance or that could necessitate epi-pen usage will not be able to ride the district-provided buses without a parent or guardian present.
Family members, or adult-accompanied minors over the age of 12, can go with you to receive their second vaccination if they accompanied you on the previous dates. Simply have them register on the ALL4Oregon website and they can join you on the bus.
Masks are required at all times on the bus. No food, eating, or drinking is allowed. Social distancing will be observed: 1 person per seat and seated next to the window, unless passengers live in the same household. Windows will be open as much as possible to allow for ventilation.
To participate in District provided transportation, please do the following:
It is recommended that you sign up for a vaccination appointment on the ALL4Oregon website for a time that most closely aligns with a transportation arrival time at the Oregon Convention Center.
Have your parent/guardian sign your permission slip and be sure to bring it with you on the day of your vaccination, unless you will be accompanied by your parent/guardian.
Like last time, if you would like to self-transport, you may visit the ALL4Oregon website and schedule an appointment at your convenience. Appointments are readily available. Tri-Met passes are also available for students only. Please contact Destinee Love, destinee_love@csd28j.org if you are interested in a student Tri-Met pass.
Please note, students aged 12 to 14 are required to have parental consent in order to receive a vaccination. 12- to 14-year olds must be accompanied by either a parent, a guardian, or someone designated by the parent. 15-year-olds do not need to be accompanied and do not require consent in the state of Oregon.
If someone other than a parent or guardian accompanies the 12- to 14-year old, they will need to provide proof of parental or guardian consent. Proof of consent is either:
A signed consent form (download)
A written or typed note that includes the parent’s or guardian’s name, their relationship to the young adult, their date of birth, a statement saying they consent to the young adult being vaccinated, and their signature.
Vaccinations are not required and are being offered on a voluntary basis. We are pleased to be able to assist our students and families in receiving their vaccination against COVID-19.

- In lieu of attending elective classes 8th grade students please join us to the promotion drive-thru event.
- Your attendance for this day will be based on:
- Turning in a chromebook that day,
- Before coming to our school email your teachers or fill out an attendance form,
- Pick up your certificate packet on this day
Last Day of School for 7th Grade ONLY:
Because the official last day for 7th grade is a half day the following is the scheduled last day for each cohort.
- Last day of school for CDL & Cohort A students is Wednesday June 16, 2021
- Last day of school for Cohort B students is Thursday June 17, 2021

This is a free program to support students transitioning from 8th grade into 9th grade at Centennial High School. The program, school supplies, meals and field trips are all free for every accepted student. Participating students who attend every day can earn 0.5 credits towards high school graduation! All students will learn high school math and writing skills, learn about how to be successful in high school and explore options for careers and college after high school!

Important!!! 8th Grade Promotion 2021
Hello 8th grade CMS families!
The end of the year is quickly approaching, and our 8th grade students are getting excited about the start of a new chapter in their lives. Our CMS class of 2021 has had a middle school experience unlike any other. They have persevered through historic obstacles with flexibility and grit. We want to celebrate the resilience and adaptability they have shown these past two years. Please join the Centennial staff for a Covid-safe, drive thru Promotion Ceremony on Wednesday, June 16th. Your student will be presented with a Certificate of Recognition and there will be an opportunity for photos.
Each 8th grade team will have its own designated time for the ceremony.
Eagles: 10:30-11:30
Foxes: 11:45-12:45
Herons: 1:00-2:00
CVA: 1:00-2:00
In order to ensure a smooth traffic flow, please enter the CMS parking lot through the entrance on 182nd Ave. Cars will only be allowed to move in one direction through the lot.
We know how proud you are of your student(s). If you would like to decorate your car with window paint and signs to show off your pride, we would love to see your spirit.

There are a limited number of spaces in the program, so if you are interested in having your student considered to be selected for the CSD Middle School Summer Programming, please fill out the Middle School Summer Programming Registration on the CSD website at https://www.csd28j.org/Page/894 or click on the online registration link below.
You can also download and fill out the Middle School Summer Program Registration on paper and return to your student’s school.
All Middle School Summer Programming registration requests are due by June 7th at 4 PM.
Priority will be given to students who had minimal attendance or engagement during the 2020-21 school year. Get your student’s Middle School Summer Programming in as soon as possible so they can be considered for the limited number of spaces we have available for this great summer learning experience.
