Parent Communication Form

Is your child leaving early for an appointment? Is someone else picking up your student? Heading on vacation?

Please send in a PARENT COMMUNICATION FORM to the front office.

If your child will be leaving early from school, he/she should visit the front office when they arrive at school to have the form signed. This form serves as his/her pass out of class, helps the attendance office staff keep track of the absences, and ensures your student is in the front office waiting for you when YOU need to pick him/her up. It's a Win-Win. As a reminder, the high school follows this process and requires these forms, too.

Students may pick up these passes in the front office (preferred) or you may print off the FORM and send it with your child.

Thank you.

District Welness

We are looking for individuals (parents OR school staff) for our district wellness committee for next school year. We normally meet 4-6 times per year to discuss and plan events for student and staff wellness for the district.

We are also looking for individuals who want to be on our district Wellness Festival Planning Committee. This committee will meet before the end of this school year, once during the summer and then frequently next school year.

For both opportunities please contact Kelsey Warren - or 513-686-1796.
