Updating Final Forms
As in the past, we need ALL parents or guardians to log into Final Forms and update any necessary changes for our 2020-2021 school year ASAP. Also newly added is a Covid-19 Parent Agreement that must be signed before the school year begins. You are required to sign the forms at the beginning of each new school year. A signature is also required after any update or as additional forms are added.
We are asking that ALL parents of students log into FinalForms and complete their forms for the 2020-2021 school year ASAP so that potential issues/problems/questions can be addressed before our return to school in September. Some forms require a student signature.

Important Notes from Nurse D
Medication Drop Off - If your child takes a daily medication at school or has emergency medications to be dropped off, we will be doing so on August 28th from 8:00-3:00 pm. There are no visitors allowed in the building, so we will be doing so by calling the clinic and meeting the school nurse at the back door of the clinic.
Immunizations - If you are NEW to the district, welcome to E.H.Greene Intermediate School. Please provide a copy of you child’s most current immunizations.
Mask Wearing - Start practicing wearing your masks everyday for longer and longer periods of time. You can do this! Helping kids get used to masks

A note from Lisa Zelvy
Dear Families,
Sycamore Community Schools will proudly continue to provide families access to private mental health services for students in the 2020-2021 school year at E.H. Greene Intermediate.
Returning to school can activate a multitude of emotions for children. As a result of the COVID-19 pandemic, we know our students need support more than ever before.
“Social and emotional wellness is at the forefront of our return to school planning. We are grateful for our continued partnership with Cincinnati Children’s Hospital,” said Lisa Zelvy, Pupil Services Supervisor. “Through telehealth appointments and at times, in person treatment, our students can continue to access care during a time when they need it most.”
Nicole Snyder, our licensed independent therapist from Cincinnati Children’s Hospital, will continue to provide care to students and families upon her return from maternity leave on October 12.
While Nicole is out of the building, we know the need for care doesn’t stop. Our team of school-based therapists can be available to support urgent and ongoing needs during her leave. Parents, teachers, and school counselors can continue to make student referrals for service.
In Nicole’s absence, families interested in initiating or restarting services should contact their assigned school counselor, Tracey Bachman or Jennifer Asher. Additionally, families can contact Lisa Zelvy, Pupil Services Supervisor, with questions or to make a referral at zelvyl@sycamoreschools.org or (513) 686-1700.
