Healthy Kids Survey-Parent Survey

I would like to encourage you to participate in the Healthy Kids Survey. The parent version of the survey is the companion survey to the California Healthy Kids Survey (CHKS) given every other year to our 5th graders and gives you an opportunity to have your voice heard about your perceptions of student behavior and attitudes, school programs and policies, and the overall school climate as they relate to student well-being and learning. The survey is anonymous.. The results will be used to help shape conversations about improvements and initiatives as we develop future plans for Mt. Diablo Elementary School.

The survey is accessible online and takes approximately 10 minutes to complete. Please click the link below to complete. We are aiming for a 75% parent completion, if not more.

Mt. Diablo Unified needs household income information from families to maintain eligibility for supplemental state and federal funding that is used to support services for students. The District is no longer collecting income information through National School Lunch Program applications, due to the recent extension of free school meals for all students. Instead, we are asking all families to complete Household Information forms (also known as Alternate Income forms) via Homelink. If families do not fill out these forms, the District could lose millions of dollars that currently supports after school tutoring, intervention teachers, counselors and/or technology.

Last year, about 900 students qualified for the funds, bringing about $836 per child to their schools for a total of approximately $752,400 that directly benefited students. But schools will lose that funding if parents do not fill out the forms each year to qualify.

Dear Parents,

Due to issues with the supply chain, availability of key ingredients, transportation and more, Aspire Bakeries and Otis Spunkmeyer are experiencing severe delays in providing product. As a result of these delays, we will be postponing our delivery until after Thanksgiving Break and we will update you as soon as we have a confirmed delivery date. We deeply apologize for this unexpected change, and understand the logistical challenges any change in scheduling creates. We are working with Otis to get the product as quickly as possible, and appreciate your flexibility as we continue to navigate these Covid related issues.
