FPS TRANSPORTATION SURVEY

This past Wednesday, all families were sent an FPS Transportation Survey message. Please take the time to complete this survey once per child that was enrolled during this 2021-22 school year. This survey gives us data on who utilized our district transportation for state reports and the same survey also helps to communicate your bus riding needs for next year. Please click here to complete the survey before Friday, June 3rd at 11:59 pm.

Notes:

  • If your student is currently a senior, please select Graduate and Does Not Require Bussing for the 22-23 school year

If your student will be driven to and from school, please DO NOT select transportation needed as a backup plan. Transportation can be added at any time during the school year by using this Bus Route Change or Addition Form

LIBRARY BOOKS ARE DUE BACK

All elementary library books are due back on May 31st. Middle school and High school books are due back on June 3rd. Please look for any missing books in cars, under furniture, or in your child’s classroom and return them to school. Families will receive a bill over the summer for any library materials that are not returned. If items are found over the summer they can be returned to the ECC District Office and student accounts will be adjusted. If families would like to provide a replacement copy instead of paying for the item, the replacement copy may be sent to school in the fall. Thank you!

IMPORTANT IMMUNIZATION REQUIREMENTS FOR 5th GRADE STUDENTS

Dear 5th grade parents:

The Student Immunization Law requires that all students entering the 6th grade receive a dose of the Tdap vaccine. This requirement can be waived only for health, religious or personal conviction reasons. To remain compliant with the law, you must provide the month, day, and year that your child received the required immunization on the attached Student Immunization Record, or select one of the waiver options prior to October 14, 2022 and return the form to your child's school. Failure to do so may result in exclusion from school in the event of a communicable disease outbreak. If you have any questions about how to obtain the required immunizations, please email nurses@franklin.k12.wi.us.

Next steps for families:

Have your child vaccinated with the Tdap vaccine if he or she has not already received the vaccine. Record the date of the immunization in the appropriate box on the Student Immunization Record, sign the form, and return it to your child’s school. To claim a waiver for health, religious, or personal conviction reasons, follow the instructions on the Student Immunization Record and return the signed form to your child’s school.

Please click here for the Immunization Fact Sheet for parents.
