South Maple Flu Vaccine Clinic
December 3, 2020 9 am – 2 pm
The Gaylord Blue Devil Wellness Center will be onsite at South Maple Elementary to provide free flu vaccinations to any student. GBDWC, in partnership with the State of Michigan, will be offering free flu vaccinations this year. We have both the injectable flu vaccine and FluMist (nasal flu vaccination). In order to provide this vaccination, please contact the GBDWC at (989) 732-6890 to schedule an appointment for that day. A parent does not need to be present for the vaccine. We just need a consent form completed and signed and this can be dropped off at the school any time prior to the clinic. The consent can be found at-
https://www.gaylordschools.com/resources/health-and-safety/blue-devil-wellness-center
If you have completed a consent in the past (for physicals or immunizations), another one does not need to be completed. If you have any questions, please contact the GBDWC at (989) 732-6890.
- The student day will begin at 8:10AM and end at 2:25PM. Students will not be allowed to enter the building before 8:10AM.
- Students will eat breakfast and lunch in their classroom. The menus may be found on the GCS website.
- Students will have a recess break with their teacher. Students will be required to wear a mask.
- Masking at South Maple Elementary is required for all students, staff, and visitors. Details about masking requirements can be found in the Return-To-Learn Plan found on the GCS website https://www.gaylordschools.com/
