HEALTH NEWS
COVID 19 Guidelines
- Medication in school: all medications must have a signed medication form on file in the health office.
- Inhalers, Epi-pens, and insulin may be carried by a student after the health care plan has been reviewed by the school nurse. No over-the-counter medications may be in a student’s backpack.
- Hearing and Vision Screening (8th grade) is scheduled for 11/2 and 11/3.
- We need 2-3 parent volunteers to help with this event. Please email the school nurse if you are interested in helping
- Close contact notices
- It is very important that you turn in each close contact notice to the office the following school day
- A physical form is sent home with the student along with a blackboard notification from Dr. Daniels.
- NOTE: if you are emailed a close contact notification, please print out the form at home and return it to school. IF you do not have access to a printer at home, please email the completed form to the school nurse aturkbergen@fcusd.org
- Please write your child’s name on the form and select the option (carefully)
- The health office is tracking a high volume of students impacted by the COVID-19 quarantine guidelines . The form allows us to ensure all families are notified in a timely manner.
- Sports close contact notifications are different from in-school close contact notifications. If you receive a sports close contact notification, please review the notice carefully before your child returns to school.
- Health Care Plans and medical 504 plans are being updated- if you have NEW health information that was not included in your child’s registration information, please email nurse Amanda: aturkbergen@fcusd.org

Because of this unfortunate situation, all parents must complete the Volunteer Personal Automobile Use Form (one form per driver must be completed) and attach to the form a copy of your driver's license as well as the declarations page of your auto insurance. The form must be completed and turned in to your coach no later than Friday, September 3.
