Complete your family's Online Forms
Custodial parents of students in kindergarten through grade 12 must fill out all back-to-school forms using the district’s OneView Forms System. Preschool parents receive a separate mailed packet of forms. This is a separate system from Infinite Campus. Parents must create a new account in OneView Forms using the instructions below.
This system simplifies the process of filling out forms for your children because much of the information you will be asked to complete will be automatically imported from Infinite Campus. Additional information you provide will be saved in the system for next school year.
Parents will need their children’s Student IDs (the same as their lunch number) to complete this process. (Find info about Infinite Campus, including an Infinite Campus Help Desk ticket if you have difficulty accessing the system.) You also should have reviewed your child’s student handbooks, as you will be asked to confirm that you have done so as part of this process.
Go to the Pickerington Schools OneView Forms website. (A link also is available from the parent portal in Infinite Campus.)

Our staff, parent and student volunteers are here to assist you with any questions you may have. Please don’t hesitate to ask. Please note the following sessions that 10th, 11th and 12th graders are reporting on specific days and 9th graders will report during the 2 days according to the month of their birthday.

12pm-3pm: All 11th graders
9th graders with Birthdays during the months of April – June
2nd Session Wednesday August 8th

During the Sessions students and families will have an opportunity to turn in medical forms, schedule pick-up, pay fees, participate in their device pick-up and walk the building and ask questions.
Stations 1-3 will take place in Commons (Cafeteria)
Station 1: Emergency Medical Authorization Collection. Please submit your EMA form at this station to proceed through the rest of the stations. You will not be permitted to move forward without turning in this form. For complete information including instructions for creating your account and filling out forms visit: https://www.pickerington.k12.oh.us/back-to-school-2018/ and click on “Complete Your Family’s Required Forms Online.
Station 2: Schedule Pick-Up. The schedule reflects the choices made by students/parents during the registration process last year. Counselors will be on hand in room 219 to help students fix EMERGENCY SCHEDULE QUESTIONS ONLY (e.g.: I’m a senior and I don’t have an English 12 class on my schedule or I’m a student athlete and I don’t have the required 5 classes on my schedule to be eligible, etc.). Please Note that if your student’s schedule is not at this station, it will be at the Fee Collection station. Once you have paid fees, from the 17-18 school year you will be given your schedule.
Station 3: Fee Collection. You will be given your student fee bill at the time you pick up your schedule. Please feel free to go to the concession window to pay your fees today. Our staff is able to accept cash, check, or credit card to pay your school fees. Paying fees today can help to avoid delays in report cards, transcripts, etc. that can be held for non-payment.
Station 4: Device Pick-up for 12th graders all other grade levels will receive theirs during the
first week of school.
*Room 109: I-Pads for 12th graders
Station 5: That’s it…you’re done!! Feel free to explore the building to find your classrooms. Be sure to visit your locker and practice the combination.
***Note RE: Textbook distribution will occur through the classroom teacher once school begins. We will not distribute textbooks during Welcome Days this year.
We look forward to seeing you August 15th for the first day of school!!
