Bulldog Blueprint

From the Nurse
Attention All Returning Taft Students:
PHYSICALS: all students who will be participating in sports need a current physical, every year, dated after April 1st. The physicals need to be turned in to the nurse OR uploaded to Final Forms before the student can participate in the sport. This includes dance and cheerleading.
MEDICATIONS: All student medications need to be picked up from the clinic by 3pm on June 3th by the student’s parent/guardian. At this time, a new “Permission for Medication” form will be given to the parent for next year. Orders for medications are needed every year. All medications left in the clinic after June 3th will be destroyed.
Attention All 8th Graders:
Summer School Gym: Those of you taking high school gym during the summer need to turn in your physical, (dated after April 1, 2022), to the gym teacher on the 1st day of class. For students with asthma, needing to use their inhalers before or during gym, turn in the Asthma Action Care Plan to the high school nurse before gym class starts.
Physicals: All incoming freshman need to get a physical, (dated after April 1. 2022) and turn it into the high school nurse on the 1st day of school-August 10, 2022. (Those of you involved in sports will need a physical AND an IHSAA form to be completed.)
All Students Taking Medications at the High School: Remember to have the Permission for Medication sheet signed by your parents and doctor. Please have a parent turn this form and the medication into the school nurse before or on the 1st day of school-August 10, 2022.
