CAFETERIA SPECIAL DIET REQUEST
If student requires a special diet, please fill out the "Medical Statement to Request Special Meals and/or Accommodations" form found on the District website (or from link below). Also available in the front office or cafeteria.
Students will need to fill out the form and have a parent sign it then turn into Mrs. Bronte in the Cafeteria. Once this form has been turned into Mrs. Bronte, the student(s) MUST check in with her each morning they are present so special meals can be prepared and marked "reserved" for student.
If a student has a medical dietary need, please have form signed by BOTH parents and a doctor. Turn form into the Cafeteria or Child Nutrition at District Office.
Click HERE to access the form.

THOUSAND OAKS TEEN CENTER
VONS SHOPPING CENTER STUDENT BEHAVIOR
We have received complaints from the Vons shopping center (Starbucks, Vons, Taco Bell, etc) about poor student behavior after school. They are leaving trash and loitering in front of businesses.
Please remind your students to be respectful towards workers and customers and clean up their trash while at the center.
