A Message to All Current TGMS 6th Grade Students
From our School Nurse:
Per the Ohio Department of Health, all students entering grade 7 in 2023 must show proof of vaccination against tetanus, diphtheria, and pertussis (Tdap vaccine) and meningococcal disease (MCV4 vaccine). If your child has already received these vaccines, please send an updated immunization record to the school nurse, Tami Tackett, RN. The record can be faxed to 614-834-6420 or emailed to tami_tackett@plsd.us. Please see attached letter.
You may take the letter (located below) with you to your child’s doctor to use as documentation of these vaccinations, or you may submit an immunization report with the dates of both vaccinations. Please return this documentation as soon as possible to your child’s school nurse. If this is not provided during the 6th grade school year, you will need to send this information to the junior high that your child will attend prior to the beginning of next school year.
The Nationwide Children’s Hospital Mobile Clinic comes to some of the PLSD schools on certain Wednesdays of each month. Immunizations can be completed at the NCH Mobile Clinic, but know that parents are responsible for taking their child to the Mobile Unit for this service. Currently, the Mobile Clinic is scheduled to be at Harmon Middle School on Wednesday, February 1st and Tussing Elementary on Wednesday, February 8th.
If you are interested in this service, please contact your child’s school nurse at:
614-834-6408 or at the email above.
Thank you,
Tami Tackett, BSN, RN, School Nurse
