Last Day to Order Birthday Ice Cream - May 8
The last day to order Birthday Ice Cream for this school year is Monday, May 8. We will continue to distribute ice cream until the last day of school, however we need order placed to ensure there is enough ice cream in stock.
The cost is $35 per class/$15 for SI and Pre-K classes. Birthday ice cream may not be purchased using money on your child's lunch account, but instead is purchased using your Mypaymentsplus.com account (under the Activities tab). Please purchase birthday ice cream at least 2 school days in advance of when you would like the class to have the ice cream.

Notes from Nurse G
**Important Information**
Picking Up Medication
If your child has medication in the clinic, it must be picked up by an adult before 10:30 AM on May 25, 2023 (last day of school.) Medications may be picked up during school hours, it is best to come in the mornings between 7:10 am – 8:00 am, as that is when the clinic is the slowest. Please understand if I am with a student at the time of your arrival, I will not be able to return the medication to you until I have completed that student’s care. If for some reason, you are unable to pick up before the last day of school, please let me know before May 15 so we can make arrangements accordingly. Any medications left in the clinic AFTER 10:30AM on May 25 will be disposed of per county and FDA regulations.
Student Health Conditions for 23-24 School Year
Health condition care plans are only valid for one school year. New care plan documents will be required for the 23-24 school year. These are available on the New Hope website. Once on the website, go to the Student Services tab and select School Nurse. When my page opens, click on forms and you may will be able to find the health condition form appropriate for your child. (Or click here to go straight to the forms page) If medication is needed for your child, a Request for Administration of Medication form must be completed (one for each medication). You may print these and drop them off or send them electronically to your child’s physician. Please make sure all forms are signed and dated AFTER Memorial Day (May 29) to be valid for the 23-24 school year. These forms are the same forms needed for all grades, so if you have a rising 6th grader with health concerns, you may use these same forms for middle school. Please bring the completed forms and medications to the clinic at Open House.
As always if you have any questions or concerns please do not hesitate to email me at Gsampson@forsyth.k12.ga.us .
