Please remember to send a completed "Medication Authorization Form" (needs provider signature) with appropriately labeled medication if your student takes any type of medication during the school day. If your student plans to self carry their medication the "Self Carry" Contract also needs to be completed.

For students with medical conditions, an individualized Health Care Plan is also needed. These are specific to conditions and will require your medical providers to complete and sign.
