Kindergarten Registration Information
The Clear Fork Elementary Schools will be holding Kindergarten Screening the following dates:
Wednesday, May 4, 2022 for Butler Elementary Students from 8:00-3:00 at:
River of Life Community Church
111 W Elm Street
Butler, OH 44822
Thursday, May 5, 2022 for Bellville Elementary Students from 8:00-3:00 at:
River of Life Community Church
111 W Elm Street
Butler, OH 44822
All Kindergarteners MUST BE 5 YEARS OLD on or before AUGUST 1, 2022.
Pre-registration is REQUIRED. Click HERE for the form. Forms can also be found at the elementary buildings, post offices, and libraries. The due date for the application is April 8th. Please complete the application and return it to your child’s school where you will receive a registration packet. Once we receive your completed application and registration packet your child will be scheduled for a screening time on one of the above dates.
Items that you MUST provide for registration:
COPY of the child’s birth certificate,
Social security card/or number
Immunization records
Residency verification
Custody papers (if applicable)

The PDF letter below was sent home with the Butler Band students prior to spring break.
The details are as follows:
5th graders will visit the HS on Friday, May 6th to practice as a whole group. They will return to Butler by lunchtime.
The concert will be Monday, May 16th at 7:00 p.m.
