2019 Medical Plan Comparison
ZCS has a self-funded health insurance plan where Unified Group Services is the third party administrator for all medical claims. St. Vincent Health has partnered with ZCS to create the ZCS Preferred Provider Network which includes physicians and facilities affiliated with St. Vincent, Riverview, or Witham Health Services as posted on the Employee Portal.
Below is the 2019 Medical Plan Options as well as 2019 Benefit Premium Rates for the Silver Plan and Eagle Plan. Click the image to view.

Dental and Vision
Dental insurance is still provided by Delta Dental and vision insurance is still provided by VSP (Vision Service Plan).
Health Savings Account (HSA)
A health savings account (HSA) is a tax-advantaged checking account that gives the employee the ability to save for future medical expenses or pay current ones. It is individually owned; however, electing to designate an authorized signer who may also withdraw funds and be issued a debit card (e.g. a spouse) is an option.
Retirees will also need to renew their enrollment in either the Silver Plan or Eagle Plan for 2019. The Green Plan is no longer an option. If a retiree enrolls in the Eagle Plan, an HSA account will need to be opened with The HSA Authority; however, retirees will not receive contributions from ZCS.
Retirees and employees can make contributions into an HSA. The annual maximum allowable contributions to an HSA as established by the IRS for 2019 is $3,500 for Employee Only and $7,000 for Family which includes Employee+Spouse and Employee+Child(ren).
Flexible Spending Account (FSA)
The Flexible Spending Account (FSA) with Nyhart will still be an option under the Silver Plan. This is not an option under the Eagle Plan.
A FSA allows the use of pre-tax dollars to pay for eligible expenses. All claims must occur during the plan year from January 1, 2019, through December 31, 2019.
Check out eligible medical expenses and calculate anticipated health expenses to determine if a health FSA is the right option. Eligible expenses includes those incurred by the employee and spouse/children.
Review eligible dependent care expenses including preschool, summer day camp, before or after school programs, and child or adult daycare to determine if a dependent care FSA if the right option.
Funds not used during the plan year are forfeited.
Allstate Supplemental Benefits
Employees are able to purchase individual critical illness, accident, universal life, and short-term disability policies through Allstate. The premiums for these policies will be payroll deducted and effective for the plan year January 1, 2019, through December 31, 2019.
Madison National Life Insurance
Zionsville Community Schools provides $50,000 in term life insurance to certified and classified benefit eligible employees through Madison National Life. During Open Enrollment, the opportunity to elect in supplemental life insurance for the employee and/or for eligible dependent(s). If an employee elects additional life insurance, he or she will be requested to complete the Evidence of Insurability form (EOI) which will be in the forms tab of the Open Enrollment site.
Review Current Benefit Election
Visit and log in to the benefits enrollment site to review current benefit coverage and update beneficiary information. This is available to all employees 365 days per year.
Employee Number is full Social Security Number. PIN is the last four digits of SSN then two digit birth year (e.g. born 3/1/1980 and SSN is 988-78-9999. PIN is 999980.).
