HEC needs your help! Please complete parent survey for the state report card.

The results of this survey are very important to HEC. We use the information to learn and grow, and it's used in our State Report Card rating. We are always proud of our HEC family!

For parents with a Parent Portal Account using a computer:

  • Log into Parent Portal
  • Select the forms link on the left-hand navigation bar
  • Select General Forms tab
  • Select "Parent School Climate Survey"
  • After clicking on the "Parent School Climate Survey,' there will be a link to the survey questions. Click this link. Please be sure to hit submit at the end of the survey.
  • All parents of students in grades K-5 are asked to participate. Please complete the survey by April 6.
For parents with a Parent Portal Account using the mobile app:
  • **Please make sure you are in the most recent version of the app prior to logging in.
  • Locate your 'District Code' and click Submit. The district code can be found in the bottom left corner of your Parent Portal.
  • Navigate to the PowerSchool App on your mobile phone.
  • Enter your "District Code' and hit Submit.
  • Enter your Username and Password and log in to the mobile app.
  • From the Dashboard select 'More.'
  • Click on 'Forms' to access forms that have been sent to the student account.
  • State Survey forms to be completed will appear on the 'General Forms' tab.
  • Click on the form title to open the form.
  • Complete the necessary information and click 'Submit.'
For parents without a Parent Portal Account:
  • You can still contact the school to set one up. Please contact Ms. Mars, school registrar, at smars@lexrich5.org, or call 803-476-4004
  • If not, District Five will email you a QR code to access the survey.
  • Please be on the lookout for the email.
Please take a minute to complete the survey. If you have completed the survey for another school, please also complete one for your HEC student. Our families have always been there for us. We appreciate each and every one of you!
