BI-Weekly Focus: Baseline & Presentation Modified Rankin Score
I know, I know...we have focused on this one before; however, it continues to need focus.
Baseline data is hard to come by and is desperately needed to accurately score the patient's level of function prior to admission.
Vin Shen Ban shared a short video (attached) and a website with me on how to score the MRS quickly. I found it to be very helpful and hope that you may have time to check it out as well.
Having answers to the questions listed on the survey (either electronically or paper form) documented in the H&P, truly paints a clearer picture of the patient's daily functions and considerably assists in the scoring of the baseline MRS and presentation MRS, if no score is provided.
I understand with emergency/urgent type admissions you would not be concerned with baseline data. However, when the patient is stable and/or when speaking with family members, you could find out the answers to some of the questions to either score a MRS or provide enough details so that I may score it.
When a patient is having an elective procedure, the questions could be asked beforehand, while in the clinic. I continually search, scrub, and dig through clinic visits for important information as well.
