All vaccines should be completed by August 5th and submitted for approval to be enrolled in classes. Please email Nurse James (Emily.James@humbleisd.net) or Mr.Kuchta (James.Kuchta@humbleisd.net) with these documents if they have not been uploaded or if you have any questions.

2. Please help decrease the chaos the first week of school by bringing in any asthma, allergy, and/or ADHD medications and paperwork the week Aug 1st-5th.

The teachers prefer to be notified the week prior to Meet the Teacher in order to plan his/her classroom and notify classroom parents if needed of any life threatening allergies.

-Prescriptions need to have the pharmacy label that matches medication order, keep inhalers and epinephrine in their original boxes with labels.

-The medication orders need to be signed by a physician and/or all other paperwork signed by parents. (*New paperwork is needed every school year).

-Any and all medications are to be brought to school by an parent/guardian (Not the student) and will be kept in the clinic for safety reasons.
