SGMS Arrival and Dismissal
Immunizations and Health Assessments
The deadline is quickly approaching for all 7th graders this Wednesday September 15th.
All 7th graders must have the Tdap vaccine as well as the Meningococcal conjugate vaccine (MCV)
Clinics are open in High Point and Greensboro at the locations on the dates and times noted below.
The Tdap and MCV immunizations will be offered on the following schedule:
GCDPH High Point Clinic located at 501 E. Green Dr., High Point, NC 27260:
- Tuesday, Sept. 14; 4 – 6:30 p.m.
- Thursday, Sept. 16; 4 – 6:30 p.m.
- Saturday, Sept. 18; 9 a.m. – 1 p.m.
- GCDPH Greensboro Clinic located at 1100 E. Wendover Ave., Greensboro, NC 27405:
- Tuesday, Sept. 14; 4 – 6:30 p.m.
- Thursday, Sept. 16; 4 – 6:30 p.m.
- Saturday, Sept. 18; 9 a.m. – 1 p.m.
To schedule an appointment, please call 336-641-3245. Appointments are not required but are strongly encouraged. If you have health insurance, please have this information readily available when scheduling an appointment.
The fax number for Southern Guilford Middle is 336-674-4278. Please ensure your child's name is located on the document you fax so that we can enter for the correct child.
Proof of immunizations and the Health Assessment Transmittal form must be provided to your child’s school within 30 calendar days of attendance to avoid suspension.

As SGMS Knights we are creating a Culture of Excellence. Students have been reviewing what each letter in EXCEL signifies and we are looking for students to apply this to their daily school routines. Thank you to the students who are showcasing these character traits each day. We will be recognizing these leaders by team on the morning show. Please discuss with your child what these mean and how to show these in their school day.

Here are a few highlights that need special attention and review
TOPS
* Shoulders, backs, stomachs and cleavage will be covered
* No Halter/Tube tops (even with a sweater or jacket)
* Undergarments should not be visible
* Lace or Sheer shirts must be layered with shirts that adhere to the rules stated above
* Light weight jacket may be worn, but with appropriate top
PANTS, SHORTS or SKIRTS
* No Sagging
* No holes or tears above the knee
* No pajama bottoms / lounge wear / sweatpants
* Leggings, jeggings or tights may be worn, IF the skirt, dress or shirt is long enough to cover the bottom area AND extends at least to finger-tip length
* No words or images across the seat of the pants
* Shorts (at least to finger-tip length)
* Skirts/Dresses (Not shorter than 5 inches above the knee)
Athletics
Come support our SGMS Knights! Fans are welcome. Tickets are $5 for adults and $3 for students. All indoor events require masking and social distancing.
