2018-2019 School Year Forms

Dear SMA Families,

Below you will find links to forms that will be due on the first day of school, Wednesday, August 22nd. Not all paperwork is necessary, so print out only what you need. If you have any questions, feel free to contact Sarah Clariza in the front office, or phone: 858-454-2461. The breakdown is as follows:

Emergency Form: Mandatory.

Extended Care: Mandatory, even if you do not think your child will use this service.

FISH/Service Hours: Mandatory. Service Hours Tally (25 hours per family, or 12 for single parents) will be required from all families by the end of the school year. The tally sheet is attached, but should not be returned to SMA until you have completed all your service hours. NEW***You may also use School Speak to track your service hours throughout the school year. You will find this tool under the menu "FISH Hours".

Pizza Lunch: Optional. We have an all-school pizza lunch every other Tuesday. Payment is made for the whole year up front. We are unable to accept payment on a lunch-by-lunch basis.

Milk: Optional. Milk is provided on all full days of school, except Pizza Lunch days, on which a drink is already provided with the lunch.

Walking Pass: Optional, for 6th 7th and 8th grades only.

Walking Field Trip: For all grades. (2 forms)

Medication: Only if applicable.

Allergy Packet: Only if your student has any food or other serious allergies.

Again, please return all paperwork on the first day of school,

August 22nd OR they can be turned in EARLY via email.

Late submittals require extra work on the part of volunteers and staff.

SMA Uniform Regulations

Uniforms are available for purchase at Dennis Uniforms. Also, all uniform items (except the skirt, skort, or jumper) are available at LandsEnd.

The office also has gently worn used uniforms for $5.00 each. Feel free to stop by the office during the summer to take a look. Summer office hours is 8:00-1:00.

Please bring all supplies on the first day of school directly to your child's classrooms.

You may drop copy paper in the school office before the first day of school.

Please contact your child's teacher with any questions regarding the School Supply List.

School Speak

Current SMA Familes: Please login to your School Speak account and make any necessary changes/corrections to your contact information.

New SMA Families: School Speak is a program we use to communicate with our families on classroom/school events, read about your child's assignments, track your child's progress in their class, and much more. Please check your email folders for your user id and password. Also, if needed, make any corrections/changes to your contact information.

Lunch Programs

SMA offers two optional lunch programs. Please click on the links to read more about it and on how to set up your account. Lunches are delivered to the school.

Welcome to Kindergarten Gathering-Monday, August 20th 2:00pm-3:00pm (in Kindergarten classroom)

Mrs. George would like to invite all the families that are enrolled in Kindergarten for the 2018-2019 school year. This is a great opportunity to meet Mrs. George, for students to explore and get acquainted with their new classroom, and for parents to ask any questions that they may have.

Immunization Records


If your child has received a recent vaccination(s), please submit a copy of their immunization record to the school office. Thank you for your cooperation.
