5 Essentials Survey
Parent Survey
The parent window for the 5 Essentials Survey opened on October 24 and we are already at 26% participation. When need at least 20% participation to generate a report with feedback. Last year we had 45% participation and I would like to set a goal of at least 50% participation from our families. Please encourage them to participate using the link below. Families do not need an email address but need to know the county and school address. We will also have tables during Parent Teacher Conferences and Movie Night for parents to take the survey. The staff and student windows will open at a later date.http://survey.5-essentials.org/illinois/survey/parent/
Teachers
Teachers log in using their login and passcode. This information is emailed to each teacher at the beginning of the survey administration window.
I can't find my passcode. - Teachers who lose their passcode can access them by clicking on the "Forgot your passcode? Click here" link on the login page, just underneath the Passcode box. You will be taken to the "lost passcode" page. After you enter your email address, you should receive an email with the passcode information. Each time a teacher requests a new passcode, the program will generate a new passcode. Old passcodes won't work once a new one is generated.
What if I don't get a new passcode? First, check your junk mail folder. Second, contact customer support at 866.440.1874 or impact-surveys@uchicago.edu to verify that you are on your school roster. If you aren't on the roster, you can take the survey using a provisional login.
Provisional login - On the lost passcode page, you will see a link that says "Can't log in? Click here to take the provisional survey." You will be asked to enter some information to help us verify that you are eligible to take the survey, then click the "login" button. This information will ONLY be used to verify that you are a teacher at your school. We will never use this information to link survey responses to a particular teacher.
Students
Students log in using their student ID and their date of birth. Students who don't have a student ID yet or have other problems logging into the system can log in provisionally. Underneath the login page for students, click the link that says "Click here to take the provisional survey". Like the provisional login for teachers, students will be asked questions that will help us identify if this student belongs to this school. Students must login with their State ID Numbers, the list can be found below.
Articulation and IEP Meeting Schedule
ACCESS Testing
- Testing Window: January 15 – February 18, 2020
- Testing all EL students – those receiving services and refusals
- Kindergarten test – given one-on-one, subs will be provided for classroom teachers (one to two days per teacher, depending on class size). Vacancies will be created, and the buildings will be assigned the sub.
- Army Trail Kindergarten subs will be provided on 1/21, 1/28, 1/29, & 2/4
- Grades 1-5: tests are taken on-line (Except for writing in grades 1-3). No subs for classroom teachers.
- Donna will email classroom teachers reminding them of who is to take ACCESS in their class after winter break.
- EL Teachers will be testing their own students and refusals – see building principals for more specific directions and refusal students that need to be tested.
- Students need to take Reading and Listening test before anything else. After they take the Reading and Listening test, principals will run the Tier Placement Report, this will identify students' Tier for writing test.
- If you need additional paper copies of the test, let your principal know.
