NHUF Information
Attention Fifth Grade Parents
Hello 5th Grade Families!
It’s hard to believe that we are already talking about the end of the year but some planning is needed! As part of the end-of-year gift, for our fifth grade class, they will be given a school t-shirt. The shirts will be able to be used for gym next year at the middle school.
We need all the children’s shirt sizes. If you could please complete the form with the needed information, that would be great! We are asking to have the forms filled out by Monday April 3rd, end of day. We need to get the shirts ordered so we can have them in time. If responses aren’t received, we will order shirts in the most popular sizes. All 5th graders will get a shirt.

Grade 5 Yearbook Pictures
We know that you’ve received the link to submit all of your wonderful pictures of your children throughout the year at school and in their extra curricular activities. We also want your fifth grader's baby pictures for a special section of the yearbook!
The baby pictures will be submitted the same as the other pictures following the instructions below. Please make sure to note “5th grade baby picture” in the description so it’s easily separated from other submissions.
If you have any questions at all, please let me know and I’d be happy to help!
Thank you so much.
Best Wishes,
Melissa Yetter, Yearbook Advisor

Classroom Placements
Each year we work diligently to determine student placements for the following school year. Since we consider the placement process to be one of our most important tasks, we would like to share the process with you. Student placement is determined and reviewed by a team of professionals consisting of your child’s present teacher, reading specialist, guidance counselor, and the principal. Additional personnel, such as the remediation teacher and special education teachers, are consulted when appropriate. Each child’s academic, social and emotional growth and needs are discussed as well as peer relations. We strive to create classes that are heterogeneous, function as cohesive groups, and provide each child with an educational environment that will meet his or her needs. We feel our time is well spent because we also have a personal investment in each child.
In view of the time and consideration taken in the placement of your child, we request that you do not ask for a specific teacher placement. Should your child need special consideration, such as a specific type of learning environment or you wish to share information about your child’s specific needs, please write the principal a letter stating those areas that should be taken into consideration when making next year’s placement. Any written request of this nature must be submitted by April 14th. Letters that include specific teacher names and/or are received after April 14th will be returned. No requests will be considered after this date. We ask that you respect our professional judgment and be assured that we strive to make the best possible match for your child.

FROM THE NURSE’S DESK
Below you will find information regarding health room procedures. If you have any questions, please feel free to contact the health room nurses at 610-473-1770.
Medication Information: If your child requires prescription or over-the-counter medication while at school, a medication authorization form must be completed by your physician. Once completed, please submit the paperwork, then contact the health room to schedule an appointment to drop off your child’s medication. Prescription medications must be in an original pharmacy labeled container. Over-the-counter medication must be in original packaging. Please refer to the BASD website for more information.
Returning Paperwork: Please return any health room forms (Physical, Dental, Medication) via email if possible. You can scan and email the original document or email a photograph from your phone to ewismer@boyertownasd.org or ekovach@boyertownasd.org.
* Change of clothing: Please keep a change of clothing in your child’s backpack, regardless of their age. Clothing can become soiled due to a bathroom accident, fall outside in the mud, or because of spilling food/drink. The health room may not have appropriately sized clothing, so if your child does have an accident of any sort, a parent will be called to bring clothing in for the child.
