Forest Lake Elementary
Honest - Empathetic - Respectful - Own my actions
9-24-21 Principal Note
Our school health office is closely monitoring COVID-19 activity at school, and unfortunately there has been a recent increase in students at home sick with COVID-like symptoms. As of today, we have 7% of students out sick.
Beginning September 23rd we had to start requiring that all students and staff wear masks at school. This will also include school age care and preschool programs that take place at our school.
COVID-19 cases this week
As of noon on Friday, we do not have any reported positive cases at Forest Lake Elementary. We will continue to follow guidelines as spelled out in the district’s School-Based COVID-19 Mitigation Matrix.

School Meals
All breakfast & lunches are free to all students this year. Any individual milk sales at breakfast and lunch will cost money even though meals are free. Milk alone is considered a la carte and that is why there is a charge for that.
Breakfast- Breakfast menus are grab and go based. Students are welcome to grab breakfast down at the cafeteria and eat it either in the classroom.
Lunch- Students will be offered two options in our elementary schools. Students will get one hot option and one grab and go box meal daily. Students will be able to grab pre-portioned, pre-packaged, and whole fruit products on their own. Our goal is to keep limited touches on serving utensils the best we can.
We are strongly encouraging all families to complete an application for educational benefits to receive potential discounts on sports and activities, Community Education classes, and to qualify for pandemic-EBT benefits. This also helps our building in how we receive Title I funds.
In addition, other educational benefits are available for those who meet the income eligibility requirements. These benefits include discounts on musical instrument rental, activity and parking fees and other school fees.
Families can apply for education benefits online (English and Spanish available) or print and fill out a paper application. Families should complete only one application per family. Translated versions of a paper application can be found below.
Factors considered in the application process are whether your family receives Minnesota Family Investment Income (MFIP) or Supplemental Nutrition Assistance Program (SNAP), or your household size and total household income. HOUSEHOLD SIZE is considered all persons, related or unrelated: this includes parents, children, grandparents who live in your home and share living expenses. Foster children may be included in the household application as part of the household size. The TOTAL HOUSEHOLD INCOME is the income each household member received last month before taxes: this includes wages, social security, pension, unemployment, welfare, child support, alimony, and any other cash income.
On-line Free and Reduced Application (English and Spanish)
Hmong Free and Reduced Application (paper)
