SENIORS THIS BULLETIN INCLUDES IMPORTANT END OF YEAR INFORMATION!
ATTENTION SENIORS!
ALL SENIORS WILL NEED TO COMPLETE THEIR "Senior Exit Survey" PRIOR TO THE LAST DAY OF SCHOOL.
The survey is located on your MaiaLearning account. (Found in your MyApps.)
- Go to your "DASHBOARD"
- Click the "SURVEYS"
- Click the "SENIOR EXIT SURVEY"
- Complete your survey!
If you don't have it completed, your counselor or Mrs. Bradley will not sign off on your senior checkout sheet and you will have to wait in line for a signature ON YOUR LAST DAY OF SCHOOL PRIOR TO LEAVING TO THE SENIOR BBQ.
If you need help finding your survey, you can stop by the Counseling Center and see Mrs. Bradley, Mrs. K or Mr. Caruso.
FINAL TRANSCRIPTS
- Your senior exit survey includes a question where to send final transcripts.
- Mrs. Bradley will use the information from the survey to know where to send your transcripts. You do not need to email her unless you have a change.
- Your final transcripts will be sent to your college mid-June.
- Papillion La Vista High School does not give students or their parents copies of official transcripts. In order to insure the integrity of our transcripts, we send them directly to the institution you plan to attend.
DO YOU NEED FINAL TRANSCRIPTS FROM UNO OR METRO?
Credits earned through other institutions are listed on that institution’s transcript and the college you are transferring those credits to need them from the college and not PLHS.
To order a transcript, you will have to follow that institution’s procedure. This will usually incurs a small charge so have a credit card handy.
- Metro (MCC) & Nebraska Wesleyan- use the National Student Clearinghouse to handle transcript requests. Information can be found on each school’s registrar’s web page with current links. (You can link directly to Metro's page HERE.)
- UNO- go to your MavLINK account, select the Academics drop down link on the left side of the page, select Request Official Transcript.
If you are enrolled in spring term classes, be sure that you choose the option to wait to send until after the current term's grades are finalized.
DO YOU NEED YOUR IMMUNIZATION RECORDS?
If you need a copy of your immunization records, you will need to go to the school portal and download a copy of your immunizations. Mrs. Bradley will NOT be emailing those to you. You will need to do this yourself by following these steps:
- Open your PLCS student portal
- Click 'Reports'
- Click 'Health-Immunizations'
Do this now because you will lose access to your student portal and PLCS student email shortly after you graduate!
ACT SCORES
PLHS does not send ACT scores. If you need ACT scores sent to an institution you will need to log onto your ACT account and request those scores to be sent.
Sending ACT Scores
- Log in to your MyACT account and select the “Send Your Scores” option from the dashboard.
- Choose the test date for which you want to submit your report.
- Enter the identification number (code) for the college to which you want to submit your scores.
Sending ACT scores outside of the four free opportunities you get each time you register to test costs $18.50 per test date per score report. Have a credit card handy.
