5 ESSENTIALS PARENT SURVEY OPEN UNTIL MARCH 11, 2022 - WE ARE AT 16% AND WE NEED 20% TO GET A REPORT!
Parents/guardians: Please take a few minutes to complete the 5Essentials parent survey for our school. The survey is administered online by UChicago Impact and is designed to generate a detailed picture of the inner workings of Illinois schools. The survey takes approximately 10-15 minutes to complete and is available in English and Spanish. To take the survey, visit https://survey.5-essentials.org/Illinois/. Families can submit one survey for each child. The survey window will be open until March 11, 2022. Your participation in the parent survey will help us understand the conditions at our school and guide improvement. Your identity and survey responses will be kept completely confidential and will never be connected to you or your child. Thank you for your consideration and support!
As a reminder, the survey has three components: students, parents, and staff. In order for a building to receive results and have those published on the school report card, participation rates of 50% for students, 20% for parents, and 50% for staff must be met. Please note that the survey is administered by the Univ. of Chicago so the district is not able to assist with any questions about or issues with the survey.
As we have in the past,
middle school students should take the survey during XLT
4th and 5th graders should take the survey during their LMC encore
staff should take the survey once they receive an email directly from 5Essentials with their login information
parents will be encouraged and/or incentivized to take the survey by the building admin
