TMS Performing Arts Elective: Grade 5 Families
To: 2022-23 Middle school parents,
TPS is excited to let you know that next year middle and high school students will have the opportunity to participate in chorus during the school day. Ms. Laura Francis will be joining TPS as our middle and high school chorus director. Ms. Francis holds a Bachelor of Music in Music Education from UMass Amherst. She has taught and student taught at the elementary, middle, and high school levels, and has a variety of vocal music experience including musical theater. We are excited to welcome Ms. Francis to TPS and look forward to the various vocal music opportunities that she will offer our students.
For the 2022-2023 school year, TMS students will have the choice of taking one of the following three performing arts electives: Chorus, Band, or Music Exploration (guitar/ukulele/piano, music in a digital world, music history). These courses will be part of your child's Unified Arts rotation. Please review these options with your child and complete the TMS Performing Arts Elective Scheduling Form 22-23 no later than Friday, June 10th.

2022-2023 Transportation
Dear Parent / Guardian(s):
Each year we update student transportation information for the purposes of planning bus routes for the upcoming school year.
If your child(ren)'s transportation method to and from school is not changing from this year to next year, then you do not need to fill out the form and no further action is needed.
For some students, there may be changes that the administration will need to know about in order to properly schedule the bus routes.
For example, if the student:
- plans to start riding with a parent/sibling/neighbor next school year
- plans to start driving their own car next school year
- has moved/will be moving to a different Tyngsborough address before next school year
If this is the case for your child(ren), please fill out the Transportation Update Form for 2022-2023 located at the link below. If changes are necessary for multiple children, you will need to make a separate submission for each child.
Also, students who will be changing buildings from TES to TMS or from TMS to THS do not need to fill out the form unless they have a change in transportation needs as outlined above.

Thank you for your attention to this notice. If you have additional transportation questions, please email transportation@tyngsboroughps.org.
Best,
Dr. Flanagan
Update From the TES Health Office
As we approach the end of the school year I wanted to send a friendly reminder of important information for the end of this school year and planning for next year.
Student Medication
For any students with medication in the Health Office (Epi-Pens, inhalers etc) If you would like the medication returned to you for the summer, please contact me as soon as possible to let me know. Otherwise any medication that is unexpired, will be kept at the school for the next school year. Medication expiring over the summer will be sent home. Additionally, any student medication that is kept on hand as a daily medication or an as needed medication must be picked up by the last day of school or it will be destroyed. Kindly contact the health office to make a plan for any leftover medication.
Physical Examinations
Under the guidelines of the Massachusetts Department of Public Health and the Massachusetts Department of Education, it is the policy of the Tyngsborough Public School District that students are required to submit a current Physical Examination report to have on file in the nurse's office prior to entry into the preschool, kindergarten, & 4th grade. Examinations must be dated within the previous twelve months to be considered current and must include a certificate of proof of immunization. Per state requirements, proof of the following immunizations must be on file for your child. The required immunizations can be located by clicking here.
Documentation of Physical Examination is also required prior to a student's participation in competitive athletics, on an annual basis. Please do not submit Physicals to a coach. This documentation can be forwarded to the email, fax, or mailing address listed below during the summer or to the School Nurse when your child returns to school. Early submission is very helpful in avoiding delay in your child participating in try-outs and starting the season.
Health Concerns
Has your child been prescribed an EpiPen/epinephrine auto injector?
If yes, we must have an Allergy and Anaphylaxis Plan and it must be signed by both the parent and by the physician that manages your child’s allergy. If your child’s physician’s office has their own form, we will accept it, providing it has all the necessary information.
Does your child use an inhaler?
If your child uses an inhaler for any reason (asthma, sports-induced asthma, viral-induced asthma, reactive airway disease etc.) and may need to use his/her inhaler during the school day, after school (ex. sports/band/theater) or on field trips and travel opportunities, an Asthma Action Plan is required. This plan allows TPS to follow your child’s physician’s guidelines as to how to manage his/her care.
Does your child have a seizure disorder?
If yes, please have the Seizure Action Plan Seizure Action Plan completed by your child’s physician so we may keep your child safe during the school day or during school-sponsored events. We will accept an action plan utilized by your child’s physician in place of this form, provided that it meets the needs of TPS.
Will your child be taking any medication during the school day?
If yes, kindly bring the Student Medication Administration Order Form to your student’s physician for completion. This form is required for all prescription and over the counter medications to be administered at school. If your child's physician has his or her own form, it will be accepted as long as it provides all of the required information and written parental consent to administer is given.
Does your child have diabetes?
Please contact me as soon as possible to discuss the plan for the school year. During the school year, contact the TES Health Office as soon as possible following diagnosis.
Any other health concerns?
Please contact me as soon as possible to discuss. I will work with you to create an Individual Health Care Plan to meet the needs of your student.
Mandated Screening
Screenings are conducted in Tyngsborough Public Schools annually in accordance with the Massachusetts Department of Public Health Regulations (refer to guidelines below). Screening tests are not diagnostic in nature. They are designed simply to indicate students who may need further attention in a specific area.
If a parent/guardian does not wish to have this screening done at school, they must send:
A written request to exclude their child from participating in school screenings AND
Written documentation from the family physician documenting their results of the screening.
Screenings are performed at TES throughout the school year as the student's schedule allows.
Postural – students will be screened for scoliosis in the 5th grades. Complete privacy is afforded the students during this examination. If your child is currently under the care of a physician for scoliosis, documentation for their health record is very beneficial.
Vision and Hearing Screening– will be conducted during the school year.
Growth and Development – Height, Weight, Body Mass Index (BMI) and corresponding percentile will be calculated for students in 1st & 4th..
Parents/legal guardians will be notified of any findings of the above screenings that necessitate medical follow-up.
If you have any questions or concerns, please contact the TES Health Office via telephone at (978) 649-1990 (option 2). My fax number is 844-417-3492.
Thank you for your anticipated cooperation. Have a safe and healthy summer.
Best,
