End of year Reminders:
All medications in the nurses’ office will be returned on the last day of school. If you know that your child’s last day will be sooner, please notify the nurse so your medications can be sent home earlier.
All prescriptions and/or over-the-countermedications need new orders at the beginning of each year.
A 5th grade physical is recommended, all 4th graders should have received a form before spring break. The forms are also on line or the form from the pediatrician’s office is fine, too.
Health/Emergency Contact forms need to be filled out every year with updated phone numbers and medical concerns. Please remember to check the Tylenol/ Ibuprofen permission box at the bottom of the form.
Thanks and have a fun and safe summer.
