24-25 Class Placement Information Form (*Due Tuesday, April 23rd.)
Over the next two months, we will be creating the 2024-2025 class lists based on our best estimate of the enrollment projections and current budget information. We would like to assure you that every effort will be made to create balanced classrooms that represent a range of styles, abilities, and interests so as to achieve an optimal learning environment for all students.
The process of placing students appropriately is a complex one that involves a great deal of thought and effort on the part of the staff. Classroom teachers, special educators, school counselors, special subject teachers, administrators, and other staff members all take part in ensuring classes are well-balanced, dynamic learning communities. Classes are carefully crafted to place students with appropriate learning peers and a well-matched teacher. We take great pride in making sure this process is completed with care, and, at the same time, that it takes into account the interests of each individual child.
Factors considered include social relationships with peers as well as academic and emotional needs. While individual needs are given a high priority, they need to be considered in the context of the group. The consensus of our educators is that the dynamic of the total group is the most critical determinant of a successful classroom experience. It is important to recognize that individual needs are better met in classrooms that “work” as a whole.
As parents/guardians, you may have additional insight and information that will aid in the placement process. Your cooperation with this request will assure a fair and credible process for placement for all our students.
Please use the link below. Please be sure to notify us on the form if your child(ren) will not be returning to TES next school year. This optional form should only be completed if you have important information for us to consider through the placement process. Please note that we do not keep forms from previous years “on file”. If you have submitted a form previously and the concerns still apply, we ask that you re-submit a new form.
On behalf of the TES staff, we ask that you respect our experience and professional judgment regarding the best class assignment for your child(ren) in the coming year. Please know that specific teacher requests cannot be honored. As always, we have your child’s best interests in mind and appreciate your trust and support.
If you have any questions, please feel free to contact Kerry Cavanaugh, Principal.

TES Health Office Update
Wellness update- It is the time of year where we are seeing an increase in reported illnesses. Please review Tips for staying healthy & in school. The nurse’s office is receiving reports of the common cold, Influenza, gastrointestinal illness, strep throat, Covid, conjunctivitis, and viral illnesses. Kindly review the TPS General Illness Protocols for school attendance. The current Covid guidelines for K-12 students can be found here. Please note that Massachusetts has not adopted the recent CDC recommendation to discontinue isolation requirements. Contact your child’s doctor for further guidance.
If you receive an email from the health office and do not have the password please reply to the email and state password needed.
Additionally, please keep the health office notified of any changes in your child’s health status to include medication changes, hospitalizations, fractures, sprains, concussions, etc. Please note if you provide your child with an activity restriction note your child will not be cleared until a note is received from the doctor, unless otherwise specified on the note provided.
Medication in School- Any medication to be administered in school needs a written doctor's order and parent written permission per Mass state law. Please know this includes antibiotics, cough drops, etc.
Mandated Health Screenings- Mandated school health screenings are in progress! If your child does not pass a health screening a letter will be sent home.
Postural screening as required by MDPH for all 5th grade students will begin the last week of April and continue until completed. Please keep in mind that these screenings are State mandated and are not diagnostic. It is recommended that if you receive a referral letter, to contact your child’s pediatrician for further guidance. If your child was screened during their last well visit, you may wish to send in a copy of the exam to the Health Office so we may update your child’s records. In this case your child will NOT be screened in school. Additionally, if you choose to refuse to have your child screened in school, a written parental note is required along with proof that your child has been evaluated by a physician for each screening refused.
Physical Examination Documentation- Massachusetts state law requires physical exam documentation annually for students entering preschool, upon entering Kindergarten, and upon entering 4th grade. Additionally students new to the district are also required to provide physical exam documentation. This is in accordance with Massachusetts regulation 105 CMR 200 M.G.L.c.7ls.57. Please contact the health office if you require assistance in meeting these requirements.
Weather: Please be mindful of the weather and be sure your students dress accordingly. Students are going outside to play as much as possible. As the weather is changing your children's skin may become dryer. Please apply lotion, sunscreen, and chapstick on your child prior to school. TES staff cannot apply sunscreen.
Clothing: Please send your student in with an extra change of clothes to keep in their locker in case they need to change during the school day. Please be sure the change of clothes is weather appropriate. The health office does not always have clothing that will fit your student and in the event that your child may need an extra set of clothing, you may be called.
As always if you have any questions or concerns regarding the health and well being of your child, please contact the health office.
Sincerely, TES Nurses, 978-649-1990 ext. 2, fax 844-417-3492

The information form, accessible through the parent portal, is used to collect items such as family information, emergency contact data, and medical permissions. It is crucial that you complete this form as soon as possible.
