A Note from Mrs. Johansen
It's hard to believe that August is just days away and we are about to start a new school year! We are so excited to welcome our students and staff back into the building as we begin the 21-22 school year together!
Student schedules will be finalized and live on Infinite Campus, Thursday, August 5th in the late afternoon. Please note we do our best to ensure everyone's 1st and 2nd enrichment choices, however when classes fill, we have to go to your next choice. We have very little wiggle room in our schedule to accommodate movement between our enrichment classes. I am confident your child will enjoy any enrichment class they are assigned to!
Students will be using hallway lockers this year. The lockers have combination locks on them. Your child will receive their locker and combination assignment on the first day of school. Locker assignments will be near their first period classroom.
If you have combination locks at home that your child can practice with, I would strongly encourage this to happen. It'll help our teachers on the first few days as we teach students how to get into their lockers and their gym lockers.
If students bring cell phones to school, they will be required to keep them locked in their locker during the entire school day.

Students only need to bring in a few items the first day, such as a notebook, pen and pencil. the Sandburg staff will tell students on what they need beyond the first day of school.

Walk Your Schedule Day!
12:30-1:15 8th grade students
1:30-2:15 7th grade students
In addition to Walk Your Schedule day, the building will be open on Monday, August 16th from 3:15-5:00 for parents and students to come in to walk your child's schedule. The classrooms will be closed during this time, but students are welcome to practice walking their schedule, if that helps ease any anxiety about coming into school for the first day.
D15 Partnership with SHIELD Illinois
For the 2021-22 school year, District 15 is partnering with SHIELD Illinois, a company that specializes in helping schools use the University of Illinois’ COVID test as part of an early screening protocol. SHIELD is a non-invasive test that will allow us to identify pre-symptomatic and asymptomatic staff and students, which will help reduce the spread of COVID-19 in our schools. Surveillance testing, in conjunction with other mitigation measures, has been successfully used by professional sports teams and other organizations to limit the spread of COVID.
The test takes less than a minute to complete. Early identification of COVID by using SHIELD will allow us to limit the number of students required to quarantine following exposure and therefore decrease the amount of time students will miss instruction in school.
District 15 plans to implement SHIELD testing when our students return in the fall as an additional layer of safety within our schools. SHIELD testing is strongly encouraged, but is optional. Therefore, we ask that each family complete a form in Infinite Campus to opt in/opt out of this additional safety measure. On this form, we ask that you indicate if you agree to have your child(ren) participate in SHIELD testing or not. Testing will take place at each school one time per week and at no cost to families.
Parent/Guardian Action Needed — Opt in OR Opt Out
Again, SHIELD testing is an optional screening that parents need to opt in or out of. All families should fill out the SHIELD consent form in Infinite Campus. Details about the consent or refusal can be found in this document. Please click on this link to complete the consent form in Infinite Campus. We ask that you make your decision no later than July 30, 2021, so we are ready to begin screening once school begins. Therefore, your immediate response is greatly appreciated.
SHIELD results are viewable by District 15 school nurses and as with all medical information, test results are highly confidential and treated as all other student records within our district. Families will be notified by the school nurse if their student’s test results are positive and will be guided through next steps, following the Illinois Department of Public Health (IDPH) guidance.

A note from the Sandburg Health Office
Hello!
I hope everyone is enjoying their summer so far! With the start of the new school year only a few weeks away, it is time to make sure your child’s health requirements are up to date.
- Annual health examinations are required for interscholastic sports participation. Physical exams are required- before trying out- for participation in an interscholastic sport. The exam form, completed and signed by a doctor, must be dated within 13 months prior to the start of practice or tryouts, and on file in the school nurse's office. Physical forms are available in the school office, online, and here: Certificate of Child Health Examination.
- All students new to District 15 will require a current physical exam with all immunizations up to date (Certificate of Child Health Examination), and students new to the State of Illinois will require an eye examination. This form is available in the school office, online, and here: State of Illinois Eye Examination Report.
- All students must provide proof of immunizations by October 15, 2021, or the student will be excluded from school. The list of current immunizations can be found on the district website here: IDPH Health Requirements.
- Students with health conditions such as asthma, epilepsy/seizures, severe allergies, and diabetes require the emergency action plan completed by their physician along with the Medication Administration Forms, signed by the physician and parent, for any medications required at school. Please bring these forms and medications on or before the first day of school. These forms are available in the school office, online, and here

- Medication Administration Form
- Diabetic Medical Management Plans must be brought in along with medication and supplies prior to or on the first day of school in order for the student to attend.
If your child requires any of these health documents and you have not done so already, please make an appointment with your doctor as soon as possible.
