Assisted Living,
An assisted living residence or assisted living facility is a housing facility for people with disabilities.
The services provided in an assisted living facility include various minor health care services as well as daily living assistance.
Requirements,
- Bachelor's Degree in Human Services Field and 2-3 years of direct experience working with the geriatric population.
- Provide facility tours and handle the inquiry process for potential residents and their families.
- Assist with the admission of residents and any required admission paperwork. Market the facility and services available within the Columbine Health Systems continuum.
- Will schedule and ensure Board and Care Plan is complete and is reviewed annually or as needed; will coordinate Care Plan Conferences and facility meetings as needed; document specific goals, objectives, and progress information for each resident concerning social services, as required.
- Upholds the Resident’s Rights and explains the Resident’s Bill of Rights to each new resident on admission, and then again annually. Counsels with the family and resident; seeks their involvement in the care planning of the resident , and seeks solutions to problems that may arise.
- Assists in the development of programs/services provided/offered at the skilled nursing facility for residents.
- Coordinate with all departments for effectively meeting the needs of residents.
- Facilitates appropriate discharge planning and referrals.
