Sample Meeting Notice
Inviting outside agencies to meetings
When booking your meetings make sure that you let me know what outside agency you want invited in the text box. I will forward your Outlook invite to the outside agencies. Example statement to put in textbox: (Robin- please invite SCC).

I will also reply to your invite with a confirmation of the outside agency being invited and a reminder about getting releases signed and listing agency on meeting notice.
Note:
- Lewis & Clark: They are not an outside agency, but you will invite Kristin Brent to a meeting for students attending Lewis and Clark. She will be listed on the meeting notice. Kristen can also be invited to a sophomore’s meeting that is thinking about or has applied to Lewis and Clark for the next school year.

Release of Information Guidelines
Even though the notice says it is good for one year, according to DESE's guidelines, FHSD was instructed during our last MISP Review that Release of Information forms must be signed at every meeting an outside agency is invited to attend. So if you have three IEP meetings, or 1 IEP meeting and 2 transition planning meetings, releases must be dated and signed before each meeting.
If the student is 18 years old, you can have the student sign the release before or the same day that you created the meeting notice.
- When you contact the parent to set up the meeting, let them know which agency/agencies you will be inviting and let them know that you will be sending home a Release of Information for each agency.
- Date the Release of Information on the day you contact the parent.
- Make a copy of the release.
- Send the Meeting Notice and the Release of Information forms home
- Hopefully the releases will be returned signed before the meeting
- If the parent does not return the releases, present them the copy of the releases you previously sent home at the beginning of the meeting. The release must be dated before the actual meeting date.
What Can DVR do for My Students?
DVR provides a variety of services:
- offer co-op elective credit for students who are working
- assist with career investigation
- assist with looking for a college or a particular program at a certain college
- assist with paying for college (financial guidelines may apply)
- provide job leads
- practice interviewing skills
- practice/reinforce basic employability skills
- assist with completing transition assessments to determine career interests
- approve Discovery & Exploration - to assist with determining employment readiness and to allow students to job shadow in a variety of job shadowing opportunities
- provide job coaching when a student requires that level of support on the job
- for Juniors - 6 weeks of paid Summer Employment
- funding for job placement services
- funds the PRETS Program (Patrick Brady) to develop pre-employment skills in sophomores and juniors
