Attention 6th grade parents
The Indiana State Department of Health requires additional immunizations for students entering 6th grade. Please be aware that requirements have changed for this year. Please check your child’s health account in Skyward, or contact the FCI school nurse, Carol Gump, if you are unsure of your child's immunization status.
The 6thgrade requirements are as follows:
- Tdap (Tetanus diphtheria & acellular pertussis)
- Meningococcal conjugate
- Hepatitis A
Hepatitis A is a new requirement this year by the Indiana Department of Health. Students that do not have the Hepatitis A series need to get the first dose as soon as possible. The minimum interval between the first and second dose of Hepatitis A is six calendar months.
If your child is out of compliance, but scheduled to receive any of the above vaccines, please let our school nurse know the date of the appointment. Students are permitted to attend school with a future appointment date on record in the clinic. Any student who has not met these requirements may be excluded from school.
Students with a medical or religious exemption on file need a new form completed each year.
FCI school nurse, Carol Gump, can be reached during normal school hours to answer any questions you have about your child’s immunization status.

Response-to-Instruction (RtI)
RtI is a general education initiative designed to provide high quality instruction and interventions matches to student needs, and using learning rate over time and level of performance to make important educational decisions. The RtI model is based on a three-tiered model of service delivery with a focus on early intervention and prevention of academic and behavioral concerns. Data collected during this process provides valuable information regarding students’ educational progress and response to interventions.
Students who do not demonstrate a response to well-implemented, scientifically-based interventions at the Tier 3 level would be referred for an educational evaluation to determine eligibility for special education. RtI is a component of HSE’s Child Find Process.
