5th Grade Consent for CA Healthy Kids Survey (CHKS)

Please complete the survey for your 5th Grader by March 24

Parent/Guardians of 5th Grade Students,

Your 5th grade student is being asked to be a part of our school’s California Healthy Kids Survey (CHKS), sponsored by the California Department of Education (CDE). This is a very important survey that will help promote better health and well-being among our youth, improve the school learning environment, and combat problems such as drug abuse and violence. Your child does not have to take the survey. Participation is voluntary and requires your permission.

Survey Content. The survey gathers information on developmental supports provided to youth; school connectedness and barriers to learning, as well as behaviors such as physical activity and nutritional habits; alcohol, tobacco, and other drug use; and school safety.

You may examine the questionnaire in the school office or at your district’s website https://sandiegounified.org/departments/counseling_and_guidance/cal-schls.

The results from this survey are compiled into the district- and county-level CHKS Reports. To view a copy of your district’s report, go to https://calschls.org/reports-data/search-lea-reports/ (Outside Source) and type in the district name.

It is Voluntary. Students who, with your permission, agree to participate do not have to answer any questions they do not want to answer and may stop taking the survey at any time.

It is Anonymous. No names are recorded or attached to the survey forms or data. The results will be made available for analysis only under strict confidentiality controls.

Administration. The survey will be administered between February 20, 2023-March 24, 2023. March 24th is the due date.

Potential Risks. There are no known risks of physical harm to your child. Risks of psychological or social harm are very small. None have been reported in 20 years of survey administration. In rare instances, some discomfort might be experienced from the questions. The school’s counseling services will be available to answer any personal questions that may materialize.

Primetime Information Email

You may have received an email regarding Primetime for the 2023-2023 school year and were wondering what this was all about. There are plans in the works to offer the prime-time program at Gage for families that qualify. This program is free to families but there are criteria they use to determine who is offered the program first. Since all of the details have not been worked out, I cannot give you all the information you need yet. My advice to you is to sign up for the program if you are interested in the free Primetime before and after school program, but do not withdraw your name from any current after-school child care programs. I will be meeting with the program coordinator this month and will get more details at that meeting. Once I have all of the information, I will organize a parent meeting so the information can be shared with you. I will also post the information in the parent bulletin.

Family Engagement Workshops

The district's Family Engagement Department periodically holds valuable informational workshops to support our district families. Please see below for upcoming sessions:

Spring Fling is Back!!

The Gage Spring Fling (family dance) is back! Friday, March 17, 5:30-7:30 pm in the Auditorium. Gage students only (no siblings). This is not a drop-off event. One adult per family must attend. Hello Gage Community!

Each year our school strives to provide the best in educational and extracurricular resources to our students. To achieve our goals, we rely on fundraisers and the support of generous donations throughout the school year.

As the Spring Fiesta Planning Committee works hard in coordinating the Parents Night Out and Silent Auction Fundraising event on May 20th, 2023, we need the community's help in procuring additional donations to auction at the event.

Items that have typically done well at previous auctions:

· Wine Tasting Experiences

· Vacation Rentals

· Legal/Wealth Management Services

· Academic / Tutoring Sessions

· Sporting Event

· Gift Cards of Any Denomination

If you have access to goods/services that would be attractive to our community, please feel free to complete, or distribute out this form to collect donations.

We also have specific sponsorship opportunities such as title sponsor for Photo Booth, Bartender Bars, etc. Sponsors will have their name/company logo displayed on event banners displayed at the school and on all event marketing materials.

Summer School 2023

We are pleased to announce that Gage Elementary is a summer school site. Summer school will begin on Tuesday, June 20 until Friday, July 21, 2023 from 8:00 am until 1:00 pm for UTK-5th grade.


Prime time will be offered from 1:00-5:30 pm during the summer school schedule of June 20-July 21 and from 8:00 am -5:30 pm on July 24-August 2nd.

You are invited to volunteer as part of our edible garden project made possible by the Sage Garden Project Award & the PTA that brings gardening, cooking classes, and nutritional science to our students. Join us in supporting our students with a general garden education and in planting and nurturing fruit and vegetables bringing to fruition more students being likely to try – and actually enjoy – eating these healthy foods they might otherwise dismiss. Lend a hand in culinary class capitalizing on this fun with hands-on activities supporting education in nutritional science and cooking skills.

Running club reminders

  • Students should be on the track during running club: Please help us remind students that if they are on the playground/field area of the campus before the 7:43 bell they are expected to be on the track. If they do not wish to participate in running club, they should wait off campus until the entry bell at 7:43.
  • Volunteers are always needed: If you are available during running club, the PTA could always use more volunteers. Please see the PTA organizer near the basketball hoops if you are able to help.

We "ran to" Hong Kong.


Due to all of the rainy days we have lowered the mileage requirement for the field trip to 125 miles. The field trip will be on June 2nd and we will be going to Rockin' Jump. Please encourage students to keep running to meet their goals.


Remember, if you lose your scan card, you can get a replacement card on Mondays. In the meantime, you can still join us for running club.


Running Club REWARDS and eligibility based on achieving certain mileage milestones.

Each student is eligible to receive the following rewards based on their tracked mileage milestones – (another reason to remember their badge every day):


Running club volunteers will distribute the rewards to the teachers to distribute to their students, except for popsicles *.


  • 1 Foot Charm and popsicle = 10 miles
  • Ribbon = 25 miles
  • Marathon Medal = 26.2 miles
  • Ribbons for each mileage milestone = 50, 75, 100, 125, 150, 175 & beyond miles
  • Off-Site Field Trip June 2023 (location TBA) = 150+ miles by 5/25/23
  • Participation Certificates = all students that ran any mileage will be awarded a certificate


*Popsicles will be distributed on the last Friday of each month, starting Sept. 30, 2022, to all qualifying students. A Running Club Volunteer will set up a table by the flag poles to pass out popsicles 10 minutes before the bell rings. The cut-off for the accumulation of miles that will earn the students a popsicle will be the Friday before the last Friday of the month. Running Club Volunteers will place a not in the teacher's mailboxes to alert them of those students that achieved the popsicle milestone. If a student is absent on the Friday the popsicles are handed out, they will miss it, there will be no make-ups.

Please remember to be respectful to our school safety patrol and observe traffic and parking laws.

  • Let children out of your car and have them run across the street. They should always cross at a crosswalk.
  • Block the driveway of our neighbors
  • Get out of your car while in the loading zone. If you want to walk onto campus, you will need to find street parking.
  • Park in the staff parking lot, these spots are reserved for staff members
