Millsap Field Day Pre-Registration
NOVEMBER 12-16, 2019
For the safety of our students and in order to accommodate the large number of visitors that attend Field Days, we are asking that you pre-register by completing and returning this form your child’s homeroom teacher by Friday, Nov. 8th.
We are requiring everyone to check in through our Raptor Parent Access System. Pre-registering in advance will help us to facilitate this process efficiently and will hopefully help everyone avoid long lines on the day of Field Day. All visitors must bring a photo ID to school in order to attend.
Lastly, if your child has field day in the morning, they will be in their regular instructional schedule in the afternoon. Due to the number of staff needed to retrieve children and to alleviate a long line at the front desk for check-out, if your child has field day in the afternoon, we ask that you please avoid transportation changes and send your child home by their regular transportation.
Please note: School-age children/siblings will NOT be admitted. Thank you.

Guidelines for Keeping Sick Children Home from School
A sick child who is unable to participate in school in a meaningful way should be kept home to rest and recover until symptoms resolve. Keeping a sick child home also protects other children, school staff, and visiting community members from contracting an illness that can be spread from person to person.Please be aware of the following guidelines for school attendance.
- Fever: A child should remain home with fever greater than 100° F and should only return after he/she has been fever free for 24 hours without fever-reducing medicine such as Tylenol or Motrin.
- Diarrhea/Vomiting: A child with diarrhea and/or vomiting should stay at home and return to school only after being symptom free for 24 hours.
- Conjunctivitis (Pink Eye): Following a diagnosis of pink eye, a child may return to school 24 hours after the first dose of prescribed medication has been administered.
If you take your child to a healthcare provider for an illness, please be sure to request a written letter stating when your child may return to school.
Change of Clothing
Pre-K, Kindergarten, and 1st grade students should bring a change of clothes in their backpack in case of restroom emergencies. Thank you!
ALL Over-the-Counter meds must be new and unopened.
Medication is to be delivered and picked up by a parent or guardian and brought to the clinic in the original container labeled with child's name, name of the medication and directions for the time and dosage. Physician's name must be on prescription medication. Students may not carry medications - including cough drops.

Parents who wish to refer their child must follow the process described in the flyer entitled Horizons Identification Process. Please review website: http://www.cfisd.net/en/parents-students/academics/gifted-talented/ . If testing is appropriate, request a HORIZONS Parent Survey and Permission to Test form from your elementary counselor, Cindy Lopez at (281)517-6036. Return the HORIZONS Parent Survey and Permission to Test form no later than November 30th. Students will not be screened unless the parent permission form is turned in on time.
Please remember that whenever your child is absent, we must have a note from you explaining why the absence occurred.
All notes must include the child’s first and last name, the date of absence, the reason for the absence, and your signature. If you have a doctor’s excuse please attach it to your note.

BOX TOPS ARE CHANGING!!
You’ll now download the new user-friendly Box Tops App, register, add Millsap Elementary, and then scan eligible receipts containing Box Tops products!
Millsap will earn 10 cents for each Box Tops product purchased plus extra earnings for any Bonus Box Tops opportunities. The app will automatically add Box Tops to Millsap's earnings online, giving visibility to see how you are supporting your school in real-time.
Keep clipping regular Box Tops and sending them to Millsap as long as you continue to find them on packages. We will still get credit for all Box Tops submitted!
More good news: you can ‘double dip’ during the packaging transition by clipping the physical Box Tops AND scanning your receipt with those participating items.
They seem like little things that really won’t make a difference, but they DO!
