New Skyward Family Access Forms
Two new forms have been published in your Skyward Family Access account. The first is the required Acknowledgment & Consent Form that needs to be completed by each student every school year. This form has updated links to the Student Handbook and Code of Conduct.
Please log in to Skyward and click on the Acknowledgment & Consent Form (Photo below).
The second new form in Skyward is Free and Reduced-Price Lunch. Families who qualified last year for free and reduced lunch will need to complete the form again. Families who may have had a job change or financial change may want to complete the form to see if you qualify for free and reduced-price lunch. Should you wish to complete it, please log in to Skyward and click on the Free and Reduced Lunch Form (Photo below).

Nurse Notes
What to expect when your child is sent to the clinic:
When a student is sent to the clinic, Nurse Jennifer will assess for COVID symptoms which include:
fever/chills, cough, shortness of breath/difficulty breathing, fatigue, muscle or body aches, headache, new loss of taste or smell, sore throat, congestion or runny nose, nausea and/or vomiting, and diarrhea.
***NOTE: If your student has any of these symptoms while at home, please do not send them to school and contact the nurse.
This is a case by case situation. Not ALL students will be sent home just because they are sent to the nurse's office. If she determines that the student needs to go home after the assessment and if it is due to COVID symptoms, Nurse Jennifer will contact the parent. The student will be sent home with a letter that explains why the student is being sent home and the options for returning to school. The parent will then receive an email from the district stating the similar details of the letter.
In order for a student to return to school after being sent home with COVID symptoms, they must follow ONE of the three options:
1. Provide medical documentation from a physician with an alternate diagnosis.
OR
2. Provide a negative COVID test.
OR
3. Quarantine for 10 days and on return, have improved symptoms and fever free for 24 hours without fever reducing medication.
The physician's note or test results must be emailed or given to Nurse Jennifer for approval to return, and it will be forwarded to Mrs. Hillje (the COVID-19 Incident Manager), who will give the final approval to return to school. The parent will then receive an email from the district for permission to return to school.
Feel free to call or email Nurse Jennifer with any questions.
