Each school year, the Kansas State High School Activities Association requires each student athlete have completed Pre-participation Physical Evaluation and Concussion/Head Injury/Heat Illness forms on file before the student athlete can participate in FSHS-sanctioned sports.

According to KSHAA guidelines, annual physical examinations shall not be taken earlier than

May 1st preceding the school year for which it is applicable.

Free physicals will be conducted by CHC/SEK in the FSHS Auxiliary Gym on Monday, June 7th, from 8 a.m. until 3 p.m. and again on Thursday, July 29th, from 8 a.m. until 3 p.m. Below are the links for the necessary forms:

Session I at Fort Scott High School begins Monday, June 7th, through Thursday, June 24th.

Session II at Fort Scott High School begins Monday, July 5th, through Thursday, August 5th.

Boys Conditioning: 7 a.m. until 8:30 a.m.

Girls Conditioning: 8:30 a.m. until 10 a.m.
