5 Essentials Survey
You should have received an email within the last week or two to complete the 5 Essentials Survey. We would like to provide you with time to do that. Please join Julia in the LMC to complete the survey on Wednesday. Treats will be provided.
Student and Teacher Surveys take about 30 minutes
4th and 5th Grade Students must also be given time to complete the survey. Information was shared in the Branch with parents that their child's classroom teacher will share when the class will do this. Please communicate the date you plan to have your students complete it. Parents have the right to Opt-Out by printing a form from the Branch and sending it in to you. If you receive any opt-outs please place them in Erin's mailbox.

Students will need to enter their student ID and birth date to take the survey. The office will print your roster with student ideas and place it in your mailbox early next week.
In case you are wondering...
- the survey is completely confidential and voluntary
- you have the right to discontinue the survey at any time
- all teacher and staff identities are kept completely confidential - no one including the school, district or state personnel is given access to see individual responses - school administrators only have access to aggregated data
- we will not walk around the LMC to look over your shoulder, we will just hang out up front by the treats
- treats are in no way associated with the 5 Essentials and are purely because who doesn't need a sweet on a Wednesday?
