7th Grade Immunization: Face to Face & Online Learners
Hello families,
Osceola County schools require 7th-grade students to have specific vaccinations. This vaccination 680 form, from your physician, must be sent to the school. This applies to all students, even current students of ours who are moving up to 7th grade. You must present a new immunizations form. (Unless you have a religious exemption, which we must also have updated on file.) Please have these forms in before school starts to prevent delayed entry for your student.
Please drop this form off at the front desk, during our new operating hours. Monday-Thursday, 9 am-4 pm.
All students entering the seventh grade must have (1) Tetanus, Diphtheria, and Pertussis (TDAP) vaccine and all other required vaccines must be up-to-date. Please provide an updated immunization record (Florida form 680, now a white form) to the school.
Please see your health care professional for more information.
Students who are not properly immunized will NOT be allowed to enter/attend school.
***Please make sure you have this immunization completed prior to the start of school (August 10th so that when 100% face to face learning begins, your child is ready.

OPTION 1: FACE-TO-FACE
If I plan for my child(ren) to return to a traditional classroom setting when school resumes on August 10, 2020, they will commit to following the safety guidelines outlined below to protect themselves, their peers, and their teachers. I will select this option for my student(s) by July 13th.

OPTION 2: DISTANCE LEARNING MODEL provided by OSCS teachers (From Home)
If I prefer for my child(ren) to start the school year with distance learning with their assigned teacher and return to the traditional classroom when a vaccine is available or conditions further improve, I will select this option by July 13th. Digital learning will follow the traditional daily schedule with live and recorded sessions.
The school volunteer program has rolled over to the 2020/2021 school year. All previously Approved OASIS School Volunteers are now Inactive.
Please visit the link shared below and RENEW for the upcoming school year. Due to safety concerns from the Coronavirus, the Osceola Science does not know what volunteering will look like in the 2020/2021 school year, but everyone is encouraged to RENEW so we will be ready when the opportunity is available. You are required to be OASIS approved in order volunteer or enter your students classroom for any event throughout the year.
We sincerely appreciate all the times you have volunteered in the past and look forward to working with you in the future
