Documentation is Key!
Just remember to document everything if you want to be able to use it later. If it is not documented, it did not happen. :)
Do it now as opposed to later. It is fresh in your mind now. Later, too many new things have happened. Right?
Update on The Neurosurgery Registry Team
Jennifer and I are moved into our new space on the 7th floor of Sprague (CS7.518). We are diligently working on incoming patients and creating a routine for our work flow.
We are almost finished with merging the old STAR Access database and ENRG database, which will create a comprehensive list of all UTSW patients with Aneurysms in one REDCap database. YES!
Having said that, remember that some data is newly being collected and older patient records will be missing that information.
We are excited about this process and can't wait for it to be complete. Once finished, Janet & Carlos will begin to merge the old SAM (AVM) Access database into the newly designed AVM REDCap database.
So big things are happening and we are pushing through to get everything in order.
As the Neurosurgery Registry Team...
It is our job to support the Neurological Surgery & Neuro-Interventional Radiology Departments by abstracting detailed data for our databases. This bi-weekly update is a way for us to communicate with you regarding what were are or are not seeing in the EMR.
The bi-weekly update will focus specifically on data elements that are not consistently documented orrecorded.
This has been created in an effort to assist with uniformity and to streamline documentation.
Our goal is to have clean data that is useable and searchable. Having the most accurate data collected and recorded in our databases only helps you! It assists with the evaluation of current practices & techniques, publishing papers/studies, and most importantly, scoring reliable outcomes.
Without reliable outcome scores...what is the purpose of collecting data? We want to know what is working and helping our patients.
Please be mindful of the bi-weekly update and try to incorporate the information into your documentation.
