Before you are permitted to enter Lithuania, you will have to complete a Lithuania Registration Form. This is needed by all overseas residents of Lithuania, even though they may not yet have a resident visa. The Ministry of Foreign Affairs in Lithuania will allow you to acquire a visa so as to go to Lithuania, but you'll still need to fill out the registration form before you can travel. You can still use your passport as a valid form of identification
You must also indicate whether or not you need a medical insurance card. You'll be required to give details such as name, contact info, passport number, phone number, date of birth, passport expires, and payment procedures. You will also have to record any other countries which you travel to in your enrollment form, like the U.S., U.K., France, Germany, and Italy. A negative diagnosis in your wellbeing authorities' records can prevent you from getting a health travel insurance plan in Lithuania.
Your enrollment form will ask you to record all your vaccinations, including your first and third series of immunizations. In case you have gotten any other vaccinations as your first visit to the vaccination center in Lithuanian capital Vilnius, then you will need to list these also. Your registration form will define if your vaccinations need to be renewed, and you will have to indicate whether you wish to continue your treatments at the centres. For those that did not receive a vaccination certificate at the time of your first visit to the vaccination center in Vilnius, you will still have to indicate these on your registration form.
List all of your personal products. These include: passport, vaccination card, airline tickets, and some other prescriptions. You'll also have to record all your personal items that are in your luggage or on your vehicle. It's critical to indicate whether any of your prescription medications have been in your possession, such as: aspirin, vitamins, as well as blood pressure drugs.
List all your personal illnesses and ailments. You will need to describe if your illness or condition is not yet diagnosed. When it isn't a contagious disease that can spread easily, you will also have to explain if it's a particular name (i.e. strep throat). The sole infectious ailments that need an entry into the vaccination centre's database are the ones that are automatically contracted through touch, inhalation, or blood.
List your marriage/divorce details. The only entry required for this component of your Lithuania Registration Form is the name. It's also advisable to indicate whether you were previously divorced or still currently being wed. The health authorities will also have to know the titles of any children or people who are residing with you. Be sure to record the legal titles of these individuals too, including the town and town that they reside in. Each of these details must be in your enrollment form for Lithuania.
