Immunizations DCHS
Immunizations: Please ensure that your child’s immunizations are up to date for the 2021-2022 school year. Immunizations must be up to date or in the process of being brought up too date or your child will not be able to attend school per Oklahoma State Law.
There are exemptions for personal, religious or medical reasons. If this is the case, please go to the web site for the State of Oklahoma Department of Health, print and fill out the immunization exemption form. This will exempt your child from needing any future mandated vaccines for school purposes.
Please bring all immunization records to DCHS so we may enter them into your child’s record if you have not already done so. For immunizations that are in the process of being completed or if you are providing an exemption form, please drop these off at the front office as needed so that we may continue to upgrade your child’s record. If you have any questions, the school Nurse can be contacted at 405-677-5777 #4151.
Thank you! Mark J. Schalk RN, DCHS School Nurse.
