Note from the Principal

Dear families of Hazeldale,


Student Supplies/Belongings Pick Up


We will be distributing student items by last name and we will use the same format we used for Chromebook distributions.


On the day/time of your pick up, please drive up with a sheet of paper that has your student’s last name, first name and teacher name. We will hand the item to you through the car window or place your student’s bagged items on a table so you can retrieve it.


If you have Hazeldale library books and/or classroom books to return you can set those on the table near the car or hand them to the Hazeldale staff member when you come to pick up your child's belongings. Please tell the staff member who your child's teacher is if handing them classroom books/materials so we can get it to the right bin!


Here are the dates and times that we would like you to come. All items not picked up during these dates will be available in the Fall.


May 12 (Tuesday)

  • 9-11 am Last Names A-C
  • 1-3 pm Last Names D-H


May 13 (Wednesday)

  • 9-11 am Last Names I-M
  • 1-3 pm Last Names N-S


May 14 (Thursday)

  • 9-11 am Last Names T-Z
  • 1-3 pm If you missed it

In-Person Wave-By Details

During spirit week Hazeldale staff decided it would be fun to do another “in person” outreach.


On Wednesday, May 20th at 4:00 PM members of the Hazeldale staff will be standing (6 ft from each) in the car pick-up/drop off line so the Hazeldale community can come by and give us all a wave or maybe even a pool noodle high five!!


Logistics/Notes:

-It will work like car pick-up/drop off, please keep the car line moving through.

-There will be traffic congestion, please be kind and patient.

-We are encouraging masks to be worn.

-If you think you might be sick, please stay home.

-We are encouraging a drive-by wave, but if you do walk please follow all social distancing measures and be careful of cars.

-Please note, this is not a time to drop off or pick up any school supplies.


Last, but not least, here is the HAZELDALE STAFF DANCE PARTY from May 8th with Special Guest, Jim Hiller (our former/beloved Assistant Principal) It turned out a little "glitchy" - but we sure had fun, and hope it makes you sing along and laugh and smile!

Parent/Guardian Input Form for Class Placement 2020-2021

Dear families and friends of Hazeldale,


Staffing for the next school year continues from late Spring and throughout the summer. Therefore, the school is not able to announce the name of teachers assigned to a particular grade level nor announce class rosters prior to August. Therefore, students will find out whom their teacher is through a letter that is mailed home to them the first week that teachers come back to work. Teachers will email his/her class list that same week as well. Be sure our school has your updated email address!


Parents are given the opportunity to provide input to the current teachers of their children on the learning needs of their child. The teachers will use this, as well as their own information to place students in classes for next year. Placement Input Forms are available as a pdf (download from below) or as a GoogleForm Link and are due by Friday, May 22nd.


We do not accept requests for specific teachers. When current teachers place students in classes, they use many factors to create optimal class groups and pair them with teachers. Class groups are balanced by gender as well as strengths and needs of students. Additionally, teachers balance classes by Special Education needs, English as a Second Language needs, and Talented and Gifted needs. Teachers also look at the behavioral needs of students. With all of this considered, we also attempt to balance class groups by size whenever possible.


Please use the Placement Input Form (pdf or GoogleForm) if you would like to ensure that certain factors are considered. I can assure you that teachers take this task very seriously. A great deal of time and effort goes into this process and each child is individually considered and placed. Please contact me with any questions you might have about this very important process.

Kinder Registration Has Begun—Register Now!

Kindergarten registration for Fall, 2020, is open! Students must be five years of age on or before September 1st, per State Statute, and live within the attendance boundary of our school. We invite you to register early as this assists us in confirming enrollment for staffing purposes.


New student registrations can be completed online. Go to the Beaverton School District Website/Enrolling Your Child (under quick links at the bottom of the main page). Be sure to register for the 2020-2021 School year.


*If you need a paper enrollment packet, there will be a box to pick up and a box to return enrollment forms on Wednesdays from 8 am to 12 pm beginning May 13th.


Upon enrolling you will either need to upload or bring in to be copied the following in order to make your child’s registration complete:


  • completed student enrollment form (if not completed online)
  • certificate of immunization
  • birth certificate (we will make a copy of your original)
  • migrant questionnaire
  • proof of address

PANDEMIC ELECTRONIC BENEFIT TRANSFER (P-EBT) PROGRAM- APPLY NOW!

The Department of Human Services (DHS) and the Oregon Department of Education (ODE) announced last week that Oregon families with children who are eligible for free or reduced-price school meals will get cash benefits for the meals they would have received at school even if they have been accessing meals from schools during the closure. Families affected by COVID-19 may still apply for free and reduced meals for this school year.


Oregon is the recipient of a grant to provide funds to children experiencing food insecurity during this Pandemic. The Oregon Department of Education Child Nutrition Programs is supporting the Pandemic Electronic Benefits Transfer (P-EBT) program by collecting data from schools. If your child is eligible, you will be contacted shortly by the Department of Human Services (DHS). Instructions will be included to activate or refuse benefits.
