Immunizations Required for School
Attention Parents/Guardians:
The Oak Park School District has updated the K-12 School Health procedures for the 2020-21 school year. If your child requires medication, prescription or non-prescription, or a medical health plan during school hours, please visit the OPSD website at https://www.oakparkschools.org/community-parents/school-health-k-12/ and follow the steps as outlined. This should be done prior to the return of in-person teaching and learning (when determined). In the future it will be done at the beginning of each new school year or any time a medical concern occurs that requires medication and/or a medical health plan. The forms are also available in the main office of your child’s school for your convenience. If you have any questions, please contact your child’s school or the District Licensed Nurse, Victoria Ipina at 248-336-7620 or vipina@oakparkschools.org.
All OPS students must have an up-to-date immunization certificate or a valid immunization waiver to attend Oak Park Schools per Michigan law. If your child(ren) need immunizations, please use this time (virtual learning), to update the required immunizations.
IMPORTANT NOTE: Students who are not properly immunized will not be permitted to return to in-person teaching and learning until immunizations are up-to-date.
